Job Summary: Transitional Care Nurse - 4 South, a 30-hour per pay period, part-time position, providing safe, effective, patient-centered care to patients with complex medical conditions.

Key Responsibilities:

  • Deliver safe, effective, patient-centered, ethical, and evidence-based nursing care according to scope of practice and in accordance with Bruyère policies, procedures, and standards.
  • Assist patients and their loved ones to manage clinical conditions and transitions, and maximize their self-care, psychosocial, spiritual, communicative, and interpretive abilities.
  • Act as a resource for members of the care team.

Requirements:

  • Valid certificate of registration from the College of Nurses of Ontario (CNO).
  • Valid Basic Life Support (BLS) cardiopulmonary resuscitation (CPR) Level C certificate.
  • Experience in rehabilitation, geriatrics, Alternate Level of Care (ALC), and/or working with patients with dementia.
  • Gentle Persuasion Approach (GPA) training.
  • Membership in Registered Nurses' Association of Ontario (RNAO).
  • Fluent in French and English.

  • Ability to work within an interdisciplinary team.
