Job Summary

We are seeking a Registered Practical Nurse (RPN) to join our team at Southbridge Care Homes. As an RPN, you will play a vital role in providing exceptional care to our residents, working collaboratively with our interdisciplinary team to ensure the highest standards of care.

Key Responsibilities

  • Provide direct care to residents, including administering medications, wound care, and vital sign monitoring.
  • Develop and implement individualized care plans, in collaboration with the interdisciplinary team.
  • Conduct regular assessments of residents' physical and emotional needs, and make recommendations for care.
  • Collaborate with other healthcare professionals to ensure seamless care transitions and coordination.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures.

Requirements

  • Current certificate of competence from the College of Nurses of Ontario.
  • Registration/eligible for registration with the Registered Practical Nurses Association of Ontario.
  • Previous experience in long-term care, preferably in a similar role.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a team environment.
  • Ability to prioritize tasks, manage multiple responsibilities, and maintain a high level of productivity.
