About the Opportunity

We are seeking a skilled and compassionate Continuing Care Coordinator to join our team at Nova Scotia Health. As a key member of our healthcare team, you will play a vital role in assessing, planning, and coordinating care for clients in the community.

The ideal candidate will have a strong background in healthcare, with experience in case management, discharge planning, and community-based care. You will be responsible for assessing client needs, developing care plans, and coordinating services to support clients in achieving their goals.

About You
  • Bachelor's Degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies
  • Current and ongoing registration with the applicable Nova Scotia Professional Regulatory body
  • Minimum three years' experience in a community or long-term care setting
  • Demonstrated knowledge and/or experience in dedicated case management or discharge planning
  • Competent with computer use, keyboarding, and able to conduct assessments using a laptop
  • Knowledge of community agencies and resources
  • Excellent assessment, interviewing, and negotiation skills
  • Excellent verbal and written communication skills
  • Ability to work independently and co-operatively in a busy multidisciplinary situation
  • Ability to multi-task, prioritize workload, and manage time efficiently
  • Valid driver's license and access to a reliable vehicle
  • Competencies in other languages preferred; French an asset
Hours
  • Full-time, long-assignment; 70 hours bi-weekly
  • Commencing as soon as possible; ending December 2025
  • Dates are approximate and subject to change
  • Shifts may include days, evenings, weekends, and holidays
Compensation and Benefits

$41.06 - $46.75 hourly

Successful candidates may be eligible for our benefits package, which includes health, dental, travel, long-term disability, and life insurance coverage, as well as a defined benefit pension plan.
