Registered Practical Nurse, Rehabilitation Program

Bruyère is seeking a skilled Registered Practical Nurse (RPN) to join our interdisciplinary team in the Rehabilitation Program. As an RPN, you will play a vital role in delivering high-quality patient-centered care to individuals with complex medical conditions.
About the RoleAs a member of our team, you will be responsible for providing safe, effective, and evidence-based nursing care to patients. You will work collaboratively with other healthcare professionals to develop and implement individualized care plans that meet the unique needs of each patient.
Responsibilities- Deliver high-quality patient-centered care, adhering to the College of Nurses of Ontario standards and Bruyère policies and procedures.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Participate in interdisciplinary team meetings to discuss patient progress and develop care plans.
- Contribute to a culture of safety and quality improvement within the organization.
- Successful completion of RPN medication administration course or equivalent.
- Valid certificate of registration from the College of Nurses of Ontario (CNO).
- Valid Basic Life Support (BLS) cardiopulmonary resuscitation (CPR) Level C certificate.
- Successful completion of the mandatory FIM (Functional Independence Measure) certification provided by the hospital within the established timeline.
- Fluent in French and English.
- Rehabilitation Nursing Education/Certification.
- Experience in rehabilitation.
- Membership in Registered Practical Nurses' Association of Ontario (RPNAO).
