Registered Practical Nurse, Rehabilitation Program Specialist

We are seeking a highly motivated and compassionate Registered Practical Nurse to join our interdisciplinary team in our Geriatric Rehabilitation program. As a key member of our team, you will provide safe, effective, and patient-centered care to patients with complex medical conditions.
Key Responsibilities- Deliver high-quality nursing care to patients in accordance with Bruyère policies, procedures, and standards
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination
- Participate in interdisciplinary team meetings to discuss patient care plans and outcomes
- Contribute to the development and implementation of evidence-based practice guidelines
- Current registration with the College of Nurses of Ontario (CNO)
- Valid Basic Life Support (BLS) cardiopulmonary resuscitation (CPR) Level C certificate
- Successful completion of the mandatory FIM (Functional Independence Measure) certification provided by the hospital
- Intermediate computer skills and proficiency in electronic medical records
- Rehabilitation nursing education or certification
- Experience working in geriatric rehabilitation or a related field
- Fluency in French and English
