**Job Summary: Client Care Coordinator**

The Client Care Coordinator is a key role in the delivery of person-centered and recovery-oriented care, working collaboratively with the interprofessional care team to provide high-quality care to clients.

**Job Description:**

The Client Care Coordinator is responsible for providing leadership to the interprofessional team by coordinating client care activities, providing information and consultation to designated care providers and students. They supervise and provide work direction to staff, coordinate staffing coverage, provide clinical leadership, guidance and support to team members, and act as a clinical resource to the interprofessional team.

**Key Responsibilities:**

* Provide leadership to the interprofessional team by coordinating client care activities, providing information and consultation to designated care providers and students
* Supervise and provide work direction to staff, coordinate staffing coverage, provide clinical leadership, guidance and support to team members, and act as a clinical resource to the interprofessional team
* Evaluate the provision of care to clients and their families in accordance with the organization's vision and values
* Participate in the development and maintenance of safety programs, ensuring the safety and security of clients and staff
* Coordinate and participate in quality improvement and risk management activities
* Provide consultation and/or screening of clients in special circumstances and provide program information to clients, their families, and the public
* Set measurable personal performance goals and objectives within a team or individual environment

**Requirements:**

* Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM)
* Three (3) years of recent related clinical experience in the designated clinical area, including one (1) year experience in a supervisory role
* Completion of substance use certificate program as required
* Physical ability to perform the duties of the position
