About the Role

We are seeking a highly skilled and experienced Client Care Coordinator to join our team at Fraser Health Authority. As a Client Care Coordinator, you will play a critical role in ensuring the delivery of high-quality patient care and services.

Key Responsibilities
  1. Leadership and Supervision

Provide leadership and supervision to a team of healthcare professionals, ensuring that they have the necessary skills and knowledge to deliver high-quality patient care.

Quality Assurance

Develop and implement quality assurance processes to ensure that patient care meets the highest standards.

Client Care Coordination

Coordinate client care activities, ensuring that patients receive timely and effective care.

Staff Development

Develop and implement staff development programs to enhance the skills and knowledge of healthcare professionals.

Communication and Collaboration

Communicate effectively with patients, families, and healthcare professionals to ensure that patient care needs are met.

Problem-Solving and Conflict Resolution

Identify and resolve problems and conflicts that may arise in the delivery of patient care.

Compliance and Regulatory Requirements

Ensure that patient care services are delivered in compliance with regulatory requirements and organizational policies.

Requirements

To be successful in this role, you will need:

  • A current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Three (3) years of recent related clinical experience in the designated clinical area, including one (1) year experience in a supervisory role.
  • Completion of a substance use certificate program as required.
  • Skills and abilities in clinical proficiency/expertise in the designated clinical area, knowledge of community resources and health-related support services, ability to supervise and provide leadership and work direction, and ability to coordinate client assignments, in collaboration with staff.
