The Apotex Centre, Jewish Home for the Aged is a 472-bed, long-term care facility which provides a range of residential and specialized programs. The small, home-like settings on each floor provide flexible schedules for programs, meals and personal care, based on each resident's needs and preferences. Apotex residents require assistance to enable them to continue to live their lives and fulfill their goals and daily activities.

Responsibilities include but are not limited to:

  • Provides tours of Apotex to individuals considering admission to Apotex and their families
  • Liaises with Ontario Health and the care team to facilitate admission to Apotex LTC
  • Leads the resident admissions process, including the completion of relevant admission assessments
  • Provides resident and/or family psychosocial assessments, support and counseling
  • In collaboration with the inter-professional team, implements a resident centred care plan and coordinates the discharge planning process for residents
  • Coordinates care conferences with members of the inter-professional team, residents and families
  • Supports resident advisory council, as appropriate
  • Provides support/consultation to other disciplines on the team re: psychosocial issues particularly related to complex patient/family situations
  • Crisis intervention
  • Acts as a linkage and communication conduit between the family and the care team
  • Establishes community linkages and outreach with relevant agencies and other Baycrest programs and services
  • Participates in teaching and other student supervision activities for health professional students assigned to this unit

Qualifications include but are not limited to:

  • Masters of Social Work degree
  • Current and valid registration with the Ontario College of Social Workers and Social Service Workers ('OCSWSSW')
  • Background in Minimum Data Set ('MDS') assessment and understanding of derived clinical outcomes preferred
  • Excellent interpersonal, communication and problem-solving skills
  • Basic computer skills – Meditech, Microsoft Word and Excel, RMNR preferred
  • Knowledge of Jewish culture, Holy Days and festivals are an asset
  • Second language an asset, i.e. Yiddish, Hebrew, Russian or Hungarian
