Job Title: Social Worker 2

Job Summary:

The Centre for Addiction and Mental Health (CAMH) is seeking a highly skilled Social Worker 2 to join our team in the Psychosis Coordinated Care Service (PCCS). As a member of our interprofessional team, you will provide client-centered psychiatric care and relevant social work services to clients with complex needs.

Key Responsibilities:

  • Provide individualized, focused support to clients to facilitate stabilization of symptoms in a less restrictive environment.
  • Develop, implement, and evaluate interprofessional plans of client care in collaboration with the team.
  • Offer individual and group counseling support to clients to help them meet their recovery goals.
  • Liaise with other services in the Complex Care and Recovery Program and the community to coordinate care.
  • Refer and link clients to appropriate services and resources.

Requirements:

  • Masters of Social Work degree and certification with the Ontario College of Social Workers and Social Service Workers.
  • Three years' experience working in a mental health setting.
  • Strong knowledge of crisis intervention, recovery, and social work interventions.
  • Ability to work with clients of diverse backgrounds and communicate effectively with stakeholders.
