**: Reporting to the Director of Operations, the Social Worker is key in providing and supporting our tenants with connections to community supports and resources to ensure safe and continued housing. This role is responsible to create, update and evaluate individualized goal focused service plans to provide short
- and long-term support to complex tenants. This position requires a strong collaborative leader who can manage change, is tenant focused, has a good understanding of Mental Health and Addictions with a focus in Harm Reduction as it relates to maintaining safe housing. Additionally, this position is also responsible for maintaining organizational Key Performance Indicators (KPI's) and accurately reporting with variance analysis on a monthly basis. Willingness to teach others on challenges related to Mental Health and Addictions and Harm Reduction.

KEY ACCOUNTABILITIES (INCLUDE BUT NOT LIMITED TO):

  • Work closely with Property Managers, health personnel and support staff in developing, implementing, and evaluation of independent living plans for tenants identified as moderate to high risk.
  • Actively identify tenants who are having trouble living in an independent living environment and work with internal and external resources to proactively address needs and transition plans.
  • Collaborate with tenants and necessary stakeholders to create and facilitate plan to transition tenants to a more appropriate housing when necessary.
  • Support clients as they move through life adjustments and provide coaching to address challenges.
  • Provide tenants with assistance, information, and advocacy to successfully maintain their housing.
  • Assistance with creating informal support networks with individuals inside and outside of Trinity Place Foundation of Alberta.
  • In collaboration with internal and external stakeholders, work to address and resolve hinderances to tenancy for tenants (i.e., nonpayment of rent, hoarding issues, behavioral concerns, etc.)
  • Always promote safe work practices, role model and provide leadership for safe work practices and behaviors for all contractors and staff.
  • Ensure team members are aware of and adhere to the safety requirements and expectations for their buildings.
  • Participate in and support organizational OH& S program.
  • Other duties as assigned

EDUCATION AND EXPERIENCE:

  • Completion of a Bachelor of Social Work (BSW)
  • Registered Social Worker with Alberta College of Social Workers (ACSW)
  • Five (5) years of Social Work or experience in a related field and a minimum of five (5) years of experience working with vulnerable populations (seniors, mental illness, disabilities, addictions, lowincome, etc.)
  • Working knowledge of Microsoft Office programs
  • Familiarity with Point Click Care (PCC) software.
  • Valid First Aid Certificate

WORKING CONDITIONS:

This industry requires empathy for and an understanding of the needs of seniors and requires a complete and current (within six months)

Vulnerable Sector Check including a Criminal Background Check.

  • Fast paced, challenging, working environment that provides/coordinates complex services to seniors and other marginalized populations. The ability to be flexible to meet the demands of this work environment is required for success. An understanding of Addictions and Mental Health and Trauma Informed care, in a Harm Reduction philosophy is an asset. This work can be stressful, and you may encounter challenging behaviors including physical or verbal aggression. This position does require shift work.
