About the Role

The Alberta Health Services (AHS) Prehabilitation Program is seeking a highly skilled and motivated Social Worker II to join our Prehabilitation Coordination Team (PCT). As a key member of our team, you will play a vital role in delivering universal and targeted prehabilitation interventions to patients across Alberta.

Key Responsibilities
  • Provide psychosocial assessments and develop individualized treatment plans for patients and their families.
  • Deliver a range of social work services, including resource-based interventions, individual, group, and family treatment, risk screening, counseling, case management, and coordination.
  • Collaborate with interdisciplinary teams to ensure seamless transitions and discharge planning.
  • Develop and implement innovative programs and services to improve patient outcomes and healthcare system efficiency.
  • Contribute to the growth and development of the PCT team, including program development, evaluation, and quality improvement initiatives.
Requirements
  • Completion of a Bachelor's degree in Social Work.
  • Active or eligible for registration with the Alberta College of Social Workers (ACSW).
  • Previous experience working in a high-volume clinic or program setting, or call centre.
Preferred Qualifications
  • Experience working in a prehabilitation or rehabilitation setting.
  • Knowledge of prehabilitation principles and practices.
  • Strong communication and interpersonal skills.
