Job Summary:
The Social Worker III role is a senior-level position that oversees the Dermatology, Rheumatology, and General Internal Medicine/Thrombosis clinics at Richmond Road Diagnostic and Treatment Center. This advanced clinical position manages a patient population with complex chronic health conditions, requiring a high level of expertise and critical thinking.
Key Responsibilities:
- Work closely with nursing staff, leadership teams, and other members of the interdisciplinary team to provide comprehensive care to patients.
- Conduct thorough assessments to determine the type of services required, and develop individualized plans to address the needs of patients.
- Provide counseling and therapy to help patients resolve social and personal problems, and refer them to agencies that provide financial assistance, legal aid, housing, medical treatment, and other resources.
- Plan programs of assistance for clients, and ensure appropriate documentation is maintained.
- Opportunity to provide educational in-services to clinical staff, and act as a clinical lead to provide leadership in the planning, development, and delivery of services in a specific area.
Requirements:
- Master's degree in Social Work.
- Active or eligible for registration with the Alberta College of Social Workers (ACSW).
- 3 to 5 years of direct clinical experience in adult complex, chronic disease.
- Strong, demonstrated knowledge, skills, and experience in assessment, formulation, diagnosis, and treatment skills.
- Advanced critical thinking, advocacy, and collaborative problem-solving skills.
- Strong expertise in navigating disability supports, including Assured Income for the Severely Handicapped, Canada Pension Plan Disability, Employment Insurance Benefits, Disability Tax Credit, and Alberta Works.
- Effective case management skills, and ability to work effectively with the team as well as independently.
- Ability to take initiative and demonstrate reliability and flexibility.
- Excellent communication, interpersonal, and organizational skills.
- Adherence to AHS policies.
- Experience working with diverse populations.
- Proficient in using Windows, E-mail, Microsoft Office (including Excel), and typing.
- Experience with Connect Care.
Preferred Qualifications:
- Understanding in the use of ACEs.
- Experience and training in Collaborative Problem Solving.
