The Birchmount Green Project features 220 high quality, purpose built, affordable and supportive housing suites
with funding by the City of Toronto and assistance from the Canada Mortgage and Housing Corporation. Fred Victor
supports parents with dependent children and singles and couples with physical disabilities referred from City of
Toronto's CAHS (Coordinated Access to Housing & Supports)/PATHS (Priority Access to Housing and Supports), to
provide intensive case management support to identified Birchmount Green residents in HASS units, using the
Housing First, Strength base, Harm Reduction and Trauma Informed Care models to promote long-term stability in
housing & community integration.

The Housing Stabilization Case Manager works as part of a team whose role is to offer case management and
stabilization services and support to identified recently housed residents who may be experiencing a number of
challenges or need support with tenancy and maintaining housing.

What You Will Do:

  • Ensure clients receive effective housing stabilization support as outlined in the Housing First Model by the
City of Toronto.

  • Assess clients through move in, housing stabilization, and community integration to assess possible eviction
risks and concerns and proactively develop eviction prevention plans.

  • Develop collaborative relationships with the Birchmount Green Property Management, program partners
and mediate between clients and the landlord.

  • Liaise with Birchmount Property Management to ensure completion of work orders.
  • Follow up with clients about their responsibilities as tenants in order to maintain housing, including
supporting them to address non-payment of rent or other breaches of tenant obligation under the
Residential Tenancies Act.

  • Provide high quality, culturally responsive, resident centered case management utilizing evidencebased
practices of motivational interviewing, trauma-informed care, and harm reduction. Using creativity and
perseverance when necessary to engage the most challenging residents with close monitoring of the
program manager.

What You Bring to the Team:

  • Bachelor's Degree in social work or a related field or equivalent combination of education and experience.
  • Experience working with individuals & families experiencing homelessness and behavioral health issues, families
with children, trauma, social isolation and/or other complex health needs.

  • Strong commitment to learning and using Harm Reduction, Cultural Competence, Motivational Interviewing and
Brief Treatment skills in order to promote improved behavioral health among tenants.

  • Knowledge of effective Crisis Intervention techniques and ability to respond appropriately in crisis situations.
