GENERAL DESCRIPTION:

The Neighbourhood Group's (TNG) Stabilization Centre Shift Lead will support the coordination of day-to-day shift activities/operations at Toronto's first Stabilization Centre.

Working on a multi-interdisciplinary team comprised of agency partners and hospital staff, the Stabilization Shift Lead will utilize their lived experience with homelessness, mental health, and substance use to help assist, encourage and mentor clients and members of the team through sharing their personal journeys of wellness, as appropriate.

Through a trauma-informed approach, they will support intoxicated and vulnerable individuals who are diverted from local Emergency Departments by Emergency Services/Paramedics.


The Stabilization Site Shift Lead achieves this by helping clients through the delivery of informal support, de-escalating conflicts, sharing resources/information, building rapport, and by providing assistance in communicating with clients.

The Stabilization site Shift Lead will work alongside the Stabilization Site's Follow-Up Worker to assign TNG peers to accompany clients in the community, helping to ensure discharge plans from the Site are implemented.

The Stabilization Site Shift Lead will be responsible for assigning positions at the start of each shift, welcoming new team members, and liaising with Managers, Follow-Up workers, and peers.

They will ensure that service delivery remains consistent with the values and philosophies held by TNG and its peer programs.

The Stabilization Site Shift Lead will follow all policies, procedures, and requirements. This position reports to the Supervisor, Mobile Community Health Services and is part of
CUPE 2289.

MAJOR DUTIES:

  • Assign peer positions and tasks as needed during each shift, ensuring the smooth transition of these positions throughout the shift.
  • Welcome new hires (postorientation) for their first shift and orient them to the space and the tasks of the peer team
  • Attending case conferences/client support huddles with other members of the Stabilization Site multidisciplinary care team if/when needed
  • Troubleshoot challenges as they arise throughout the shift with the support of the Mobile Health Services Supervisor, and/or the Interim Senior Manager of Mobile Health Services
  • Ensure IPAC measures are adhered to by peers on shift
  • Ensuring peer team policies/procedures are followed, seeking additional support from the Stabilization Site Program Supervisor, as needed
  • Identify the need for Follow-Up and/or PSW supports
  • Collaborate with the Site partners and those doing discharge plans to connect clients with applicable resources and referrals
  • Intervene and deescalate a crisis situation
  • Maintain accurate notes/ records and complete reporting as required by funders and Supervisor, adhering to PHIPA and TNG's policies daily
  • Attend regular team meetings
  • Conduct accompaniments with clients to appointments as needed

REQUIREMENTS & QUALIFICATIONS:

  • Ability to maintain a calm, empathetic and traumainformed approach when supporting peers and clients throughout the shift, especially during or after crisis situations
  • Lived/living experience of homelessness, accessing community dropins, shelters, and/or harm reduction programs
  • Willingness to accept feedback and incorporate it into the work environment
  • Demonstrated ability to work well with others in a team environment
  • Demonstrated understanding of harm reduction and overdose response practice
  • Knowledge and utilization of antioppression and equity seeking practices
  • A good understanding of community/social responses to harm reduction needs
  • Understanding of all IPAC regulations, disinfecting requirements, donning and doffing PPE, etc.

Salary:
Up to $28.54 per hour

  • 8 hour shift

- working in community drop-ins and shelters: 1 year (preferred)
- working in harm reduction programs: 1 year (preferred)
