Position Overview:

You will be an essential advocate for patients, helping coordinate their biologic medications and therapies. What You'll Be Doing:

Patient Support and Advocacy:

  • Support patients throughout their healthcare journey.
  • Advocate for patients' needs, ensuring they have access to the necessary resources and therapies.

Coordination of Care:

  • Collaborate with SRx Health pharmacies, physicians, and healthcare providers to ensure seamless patient care.
  • Assist with insurance paperwork, applications, and appeals.
  • Develop and implement individualized care plans for patients.

Education and Support:

  • Educate patients on medical conditions and treatment options.
  • Empower patients to actively participate in their healthcare decisions.

Documentation:

  • Maintain accurate patient records and communicate progress to the healthcare team.

Compliance and Quality Assurance:

  • Ensure care meets all regulatory and quality assurance standards.
  • Adhere to PIPEDA, provincial legislation, and payer regulations.
  • Act with integrity and compliance in all internal policies and regulations at SRx Health Solutions.

Additional Duties (Including but Not Limited to):

  • Assist prescribing physicians in completing patient enrollments.
  • Liaise and advocate with third-party providers and provincial governments.
  • Schedule patients and facilitate communication with healthcare professionals.
  • Maintain accurate program records and provide timely reporting.

About You:

  • Education: Certificate in a healthcare-related field (e.g., pharmacy assistant or technician).
  • Clinical Background: Experience in a clinical setting is a plus.
  • Biologics Knowledge: Familiarity with biologic medications and therapies is an asset.
  • Communication Skills: Strong verbal and written communication skills are essential.
  • Organizational Skills: Excellent organizational skills for managing patient care efficiently.
  • Insurance Knowledge: Familiarity with health insurance processes and requirements is helpful.
  • Interpersonal Skills: Ability to work effectively with a diverse group of individuals.
  • Problem-Solving: Strong problem-solving skills are key.
  • Tech Savvy: Proficiency in healthcare information systems and electronic medical records is required.
  • Compliance: Understanding of healthcare regulations and adherence to compliance standards.
