Job Description

We are seeking a highly skilled Clinical Care Coordinator to join our team at Home and Community Care Support Services. As a Clinical Care Coordinator, you will play a vital role in providing high-quality care and support to patients in their homes.

Key Responsibilities:
  • Develop and implement comprehensive care plans to meet the unique needs of each patient.
  • Conduct thorough assessments and evaluations to identify patient needs and develop strategies for care and support.
  • Collaborate with healthcare professionals, patients, and families to ensure seamless care coordination and communication.
  • Provide education and support to patients and families on disease management, self-care, and health promotion.
  • Monitor patient progress and adjust care plans as needed to ensure optimal outcomes.
Requirements:
  • Bachelor's degree in Nursing or related field.
  • Current registration with the College of Nurses of Ontario.
  • Minimum 2-3 years of experience in a clinical setting, preferably in home and community care.
  • Fluent in verbal and written French.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
