About the Role

We are seeking a skilled Care Coordinator to join our team at Home and Community Care Support Services. As a key member of our care team, you will play a vital role in assessing and determining patient care needs and eligibility, providing access and referrals to community services, and engaging with patients, caregivers, and other healthcare practitioners.

Key Responsibilities

  • Assess patient care needs and determine eligibility for services
  • Provide access and referrals to community services
  • Engage with patients, caregivers, and other healthcare practitioners
  • Collaborate with the care team to develop and implement patient care plans

Requirements
