Job Description

We are seeking a highly skilled and compassionate Clinical Care Coordinator to join our team at Home and Community Care Support Services. As a Clinical Care Coordinator, you will play a vital role in providing high-quality care and support to patients in their homes.

Key Responsibilities:
  • Develop and implement individualized care plans to meet the unique needs of patients
  • Conduct comprehensive assessments and evaluations to identify patient needs and develop strategies for care
  • Collaborate with healthcare teams to ensure seamless transitions and coordination of care
  • Provide education and support to patients and their families to promote health and well-being
  • Monitor patient progress and adjust care plans as needed
Requirements:
  • Bachelor's degree in Nursing or related field
  • Current registration with the College of Nurses of Ontario
  • Minimum 2-3 years of experience in a clinical setting
  • Fluent in verbal and written French
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
