Your Opportunity:
If you enjoy a role in providing services to clients in a community based setting then this position will be of interest to you. The Licensed Practical Nurse (LPN) in Home Care works as a member of an interprofessional health care team utilizing the nursing process to provide direct nursing care to individuals across the life span. The LPN is responsible for her/his nursing practice as defined by the College of Licensed Practical Nurses of Alberta (CLPNA). In collaboration with the Home Care Manager, the LPN is responsible to assess, plan, implement and evaluate the needs of Home Care clients and their caregivers/families. There is also an expectation to be a mobile worker. This role requires travel within the city of Calgary including caring for clients in their homes or in the community. You will be responsible to operate a laptop computer and mobile phone, and for manual handling (lifting, lowering, carrying, pushing and pulling) as a regular part of the job. Learning resources and training are provided to safely perform work.
Description:
As a Licensed Practical Nurse (LPN), you are part of an interprofessional health care team utilizing nursing processes, critical thinking, problem solving and decision making skills. You will be guided by your education, experience, demonstrated skill level and AHS policies and procedures. You will play a key role in providing safe, quality patient and family centered care while reflecting the shared vision and values of AHS.
Required Qualifications:Completion of an accredited practical nurse education program. Active or eligible for registration and practice permit with the College of Licensed Practical Nurses of Alberta (CLPNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
Additional Required Qualifications:As Required.
Preferred Qualifications:Two years of recent home care experience in the last five years. Experience working with acute clients and/or long-term clients with a focus on chronic disease management. Experience in providing case management services. Connect Care training. PARIS training. Non-Violent Crisis Intervention – Certification. Parenteral Nutrition Qualification. PleurX Qualification. CVC Qualification. Wound, skin and ostomy care. Palliative support and client teaching.
