*RESPONSIBILITIES:*
· Assesses, diagnoses, manages and evaluates health/illness concerns related to any underlying cardiac risk factors and disease of the patients within the context of the patient’s experience and determinants of health.
. Perform physical exams and take medical histories
. Develop and implement patient care plans
. Diagnose and treat common illnesses and injuries
. Order and interpret diagnostic tests
. Prescribe medications in accordance with provincial and territorial regulations
. Monitor and manage patients with chronic illnesses such as diabetes, hypertension, and asthma
. Provide patient education about health promotion, disease prevention, and self-care management
. Collaborate with other healthcare professionals, including physicians, nurses, and allied health professionals, to optimize patient care
. Maintain accurate and up-to-date patient records
· Demonstrates advanced knowledge and synthesis of advanced nursing practice within cardiac sciences including in depth understanding of cardiac diagnostics (ECG, Echocardiograms, MPI, MRI and Angiograms)
· Performs advanced health assessment and clinical decision making within a clinical specialty for the purpose of making a diagnosis or assessing the effectiveness of treatment. E.g. Obtaining blood samples from patients, Artery screening and function testing, Clinical fitness testing.
· Orders and interprets relevant screening and diagnostic tests for health promotion and heart disease prevention counselling.
· Selects, recommends and prescribes effective interventions according to predetermined protocol (exercise, diet, lifestyle measures, and supplements). Where drugs are considered this will be in accordance with appropriate guidelines
· Communicates the results of all tests/results, assessment and treatment plan to patients.
· Liaise with patient’s primary care provider once predetermined protocol has been completed.
· Where appropriate, initiates referrals and coordinates care with other health care providers and community agencies.
· Provides counselling, education, and lifestyle recommendations
· Monitors and documents clients’ adherence with recommended treatment.
*NON-CLINICAL ASPECTS*

· Outcome tracking which may include compiling data for the purposes of research to expand knowledge in the area of health prevention and reversal of disease
· Education of patients and family.
· Providing consultation to nursing staff, managers, physicians, and other members of the multidisciplinary team regarding clinical and professional practice issues
· Shares expertise and provides consultation beyond the boundaries of the Clinic in areas of specialization
*QUALIFICATIONS:*
· Masters Degree in Nursing with a License to practice as Nurse Practitioner.
· Current practicing registration as a Nurse Practitioner with the British Columbia College of Nurses and Midwives (BCCNM)
· Functional Medicine Certification or Lifestyle Medicine considered an asset.
· ACLS certification considered an asset.
· Experience in chronic disease management, cardiology and/or internal medicine is preferred.
· Team player with a great positive attitude.

Job Types: Permanent, Part-time, Full-time

Pay: $55.00-$75.00 per hour
