ABOUT THE ROLE

This position is responsible to plan, prepare and facilitate restorative care programs for both Maple Health Centre and Newmarket Health Centre. The role focuses on the residents' continuity of care to encourage independence and opportunities to meet the social, emotional, physical, and restorative care needs of residents. As a member of a multidisciplinary team, collaborates with contracted physiotherapists, the Restorative Care Aides and other professionals, this position is responsible for leading the implementation, evaluation, and revision of the therapeutic plan of care for residents for the restorative care program.

WHAT YOU'LL BE DOING


•    Ensures that participating residents in the restorative care program receive care and assistance towards independence based on a restorative care philosophy to maximize autonomy a per the Resident Bill of Rights. 

•    Coordinates and integrates restorative care approaches into each participating resident plan of care to help maintain or improve their functional and cognitive capacities in all aspects of daily living to the extent of their abilities. 

•    Develops and documents restorative plan of care, restorative goals to reflect residents needs, preferences and current status. 

•    Ensures that the resident, and the resident's substitute decision-maker, if any, are consulted in the development, implementation, and progress of the restorative goals and plan of care. 

•    Assesses new admitted residents to determine eligibility for restorative care program.

•    Assesses the restorative plan of care and the resident's progress at weekly reviews and together with the interdisciplinary team makes recommendations to discharge the resident from the restorative program.

•    Completes monthly evaluation and ensures the resident's restorative care needs and progress are documented and implemented in a timely manner.

•    Tracks and documents residents' attendance and participation in the Restorative Care Program. 

•    Collaborates with the Physiotherapist and Occupational Therapist (OT) to develop and offer a variety of restorative activities/to residents at structured times throughout the week and weekends as per resident needs and plan of care.

•    Collaborates with RNs/RPNs/ADOCs to develop and offers a variety of nursing restorative care including Activities of Daily Living, eating techniques, bowel and bladder techniques etc.

•    Collaborates with therapists to ensure residents are properly using their assistive devices and sitting posture.

•    Liaises with nursing and therapy services to ensure timely communication between program areas.  

•    Organizes and prioritizes schedule to provide on-site support to Maple and Newmarket.  

•    Daily touch base with restorative aides and nursing team at Maple and Newmarket.

•    Provides residents with a safe and comfortable environment by being aware of risk management issues and concerns that could compromise safety or incur liability.

•    Leads program evaluations through interviews, observations, surveys to assess the effectiveness of restorative care program and future planning.


•    Responsible for the maintenance and accessibility of equipment inventory and ensures adherence to IPAC measures for cleaning equipment is in place. 

•    Leads the preparation of accurate, timely reports, documentation of services provided, and other data as required.

•    Ensures accurate and complete documentation.

•    Participates and contributes to the Continuous Quality Improvement Committee including submitting timely restorative care program data and analysis. 

•    Attends interdisciplinary care conferences; and assist in reviewing any admission applications.

•    Complies with all Regional policies including code of conduct, privacy and confidentiality, attendance. 

•    Reports incidents or complaints in a timely manner to the Director of Care/designate and ensures compliance with applicable policy and legislation. 

•    Responds to inquiries and resolves concerns, as appropriate.

•    Ensures that services provided meet Regional customer service standards.

•    Participates on committees, work groups, task forces and special projects, as assigned.

•    Manages information in accordance with legislation and corporate standards.

•    Performs other duties as assigned, in accordance with Branch and Department objectives.

WHAT WE'RE LOOKING FOR

 Successful completion of a College Diploma as a Registered Practical Nurse or relevant Nursing program 

•    Current registration with the College of Nurses of Ontario in good standing.

•    Minimum two (2) years experience in long term care or a rehabilitation centre as a direct care provider with restorative care or rehabilitation.

•    As a condition of employment, candidates will be required to provide a satisfactory Police Vulnerable Sector Check (PVSC), tuberculosis screening and proof of immunization in accordance with the Long-Term Care Homes Act (O.Reg.79/10).

•    A sound knowledge of the Fixing Long Term Care Act, 2021, Occupational Health and Safety Act and any other pertinent Federal, Provincial and Municipal legislation that influence the operation of a nursing home.

•    Ability to work with diverse residents, families, staff, and colleagues and comply with the Region's EDI policy and complete the relevant EDI educational requirements.

•    Adheres to Home IPAC policy and MLTC IPAC requirements. 

•    Demonstrated ability in the Region's core competencies.

•    Intermediate skills in virtual platforms and MS Office Suite.

•    Demonstrates ability to work independently and as an effective team member in a cooperative manner with all members of staff, families, residents, volunteers, and internal and external partners.

•    Strong report writing, research, analytical and presentation skills and work to tight deadlines.

•    Ability to travel to offsite locations, as required. 

•    Ability to work outside regular business hours including evenings, weekends, and statutory holidays, as required.
