**Context**:

Our Programs team provides a holistic model of care for Indigenous youth and seeks a positive and experienced team lead to support day-to-day guidance for front-line youth workers in coordinating culture, case management, and health promotion activities for Indigenous youth which includes diabetes prevention, healthy eating, group fitness, healthy sexuality, and after school programming.

The lead will guide the health promotion and youth team in their roles. Provide guidance in case management and health promotion and be responsible for program targets and deadlines while also assuming a hands-on role in supporting youth and adults in various health promotion activities.

**The Role**:
Reporting to the Director of Programs, the Health Promotion Team Lead’s responsibilities includes:

- **CLIENT SERVICE & SUPPORT**

The Youth Team Lead supports Wabano and the community by:

- Working with and for our Indigenous community to provide a healthy future;
- Leading and supporting team members through completing team and individual work plans, meeting goals, reporting and problem solving;
- Sourcing consultants (ex. Elders, traditional educators, facilitators), manage contracting of consultants for events planning and program delivery;
- Providing orientation for staff and consultants;
- Supporting team to provide case management and systems navigation to eligible individuals and manage referrals from Indigenous and mainstream organizations;
- Building and maintaining relationships with community partners;
- Coordinating and facilitating Indigenous programs and workshops as required;
- Conducting program evaluation and reporting.
- **RECORD KEEPING**

Document the story of the way you support the client’s wellness journey in the internal database using service and cultural codes.
- Enter all encounters in the database in a timely fashion and maintain client records in the database;
- Document any internal or external referrals to support the clients healing journey.
- Complete monthly reports using the template provided.
- **AGENCY SUPPORT**

Support some key programming as an integral part of the interdependent (interdepartmental) matrix within Wabano
- Assist with functions such as Gala, Symposium and AGA, and community events such as Pow wows.
- Participate in professional development for continued improvement of skills and knowledge.

**Qualifications and Skills**:
In addition to the team management and leadership skills, we are looking for people who have:

- A Bachelor Degree or College Diploma in health sciences or a related field;
- Experience with youth and adult programming;
- Knowledge of Indigenous peoples’ history and challenges Indigenous people face in contemporary society;
- Demonstrated superior problem-solving ability with sound judgment;
- Strong oral and written communication skills;
- Demonstrated experience working with high risk populations and managing crisis situations;
- Knowledge of available resources in Ottawa;
- Strong organizational skills and the ability to handle sensitive issues with tact and diplomacy;
- Ability to work a flexible work week that may include non-traditional hours, evenings and weekend.

**Salary**:
Range $60,000 - $65,000 per year + Benefits
