Home Coordinator

**General Description**

The Home Coordinator reports to the Hospice Director and supports Ian Anderson House’s current strategic and operational plan by ensuring all care is provided for our residents in a manner that meets the philosophy, mandate and standards of Ian Anderson House.

The HC is responsible for creating a safe and healthy work culture and leading the hospice clinical care team to ensure the delivery of quality end-of-life round-the-clock care. The HC is also responsible for performing the role of the Intake Nurse as required.

**Core Job Responsibilities**
- Build and foster a collaborative, evidence-based and best-practices work environment where team members are motivated to serve as full partners (being accountable for their own contributions while working collaboratively with others) in the provision of quality end-of-life care.
- Lead the clinical care team to ensure the delivery of quality end-of-life palliative care.
- Perform, support, and assist with the Intake Nurse role as required.
- Assist with developing, implementing, reviewing, evaluating and updating policies, procedures, and processes affecting all resident operational areas including admission; clinical care; supporting documentation; supplies/equipment; family support; and facilities management.
- Direct, organize (schedule & approve time sheets), train, develop, motivate and supervise the work of clinical staff. Assist with hiring and performance evaluation of staff.

People management responsibilities include but are not limited to:

- Providing clearly understood goals, objectives, quality standards and expected performance outcomes.
- Providing regular and ongoing feedback to ensure work is being carried out according to performance standards and expectations. Rewarding and recognizing performance and when needed taking appropriate corrective measures.
- Facilitating and supporting opportunities for staff and team training and development.
- Problem-solving and coaching staff on effectively managing conflict and intervening when appropriate.
- Assist with overseeing all documentation records and training requirements in compliance with the College of Nurses of Ontario and municipal and provincial regulations and legislation; including but not limited to: CNO Professional Standards, Therapeutic Nurse-Client Relationship and Consent; OHSA; WHMIS; Fire Safety including plans, drills, audits etc.
- Work in a safe manner with due care and attention to safety of self and all others. Ensure IAH is a safe and healthy work environment as required by the _Occupational Health and Safety Act and Regulations_. Take corrective action when needed. Complete incident reports as needed, which includes investigating, documenting and reporting incidents and injuries to staff, residents and guests and any property damage and/or loss of property.
- Supervise the process of resident admission and provision of care to the highest professional standards using an evidence-based best-practices planning approach. If required, manage resident discharge process.
- Coordinate and collaborate with physicians and multidisciplinary health care staff to ensure high-quality end-of-life care is being provided in a manner that reflects best practices as well as the philosophy and standards of IAH.
- Monitor the performance of agency staff and manage all agency and Community Care Support Services relationships to coordinate staffing, secure quality clinical resources, and manage supplies to support the philosophy and standards of IAH. Participate in value-added service-related meetings and in the development of policy and procedures.
- Ensure resident files, care plans, service records, reports and documents are completed and vital statistics recorded as per nursing and palliative care standards. Send out notification of death to Home and Community Care Support Services and all appropriate agencies (i.e. Care Partners, Acclaim Health, oxygen vendor; and bereavement support groups) Complete random file audits every three months and act upon potential risks and administrative or operational deficiencies.
- Facilitate effective and open “House” communication. Participate in staff report, identifying “House” concerns and intervening when necessary. Provide opportunities for staff debriefing.
- Maintain positive communication with the families. Keep the responsible parties of residents (SDM/POA) aware and informed of the status of their family member on a regular basis.
- Manage resident and family relations including concerns. Coordinate family meetings as needed. Escalate concerns or complaints when appropriate.

**Qualifications & Working Conditions**

The individual will possess:

- Current certificate of competence, CNO registration in good standing.
- BScN degree prepared, Masters’ degree preferred.
- Minimum 5 years of experience in Palliative Care, including end-of-life care.
- Oncology nursing experience.
- CNA Hospice Palliative Car
