About the Role
The MNO Indigenous Diabetes Prevention Program aims to improve our current response to the issue of diabetes support and prevention throughout its communities by adding to the current strategy. This will increase the MNO’s ability to provide awareness, prevention, direct support and education across the province. The Healthy Choices Diabetes Coach will work collaboratively with other MNO stakeholders to develop tools, provide client-based activities and engage is education/awareness activities to support better Diabetes outcomes for families. A personal, holistic, culturally relevant and pre-structured response program to diabetes will support a greater number of families to prevent the disease, understand the implications of diabetes in their families and to progress through the myriad levels of support more quickly and effectively. This will increase the opportunity for people to mitigate the impacts of Diabetes in their lives and will create significant improvement to the quality of life for a greater number of Métis families affected by the disease.
What you will be doing
- Ensure that Healthy Choices Diabetes program is delivered in the spirit and philosophy of the Métis Nation of Ontario’s Statement of Prime Purpose
- Create and implement a cultural, family-oriented client Diabetes Planning Program to support clients to better understand their needs and to develop and act on healthy choices
- Work with cultural stakeholders and knowledge holders to find culturally resonant approaches to promote awareness/prevention and provide direct support and symptom mitigation to people affected by diabetes
- Establish a referral network with expert Diabetes service providers in communities throughout the Province
- Work with Healing and Wellness Managers to support opportunities to engage Métis people in healthy living/diabetes prevention activities and in activities to support them to plan to mitigate the symptoms of diabetes in their lives
- Engage with clients to improve their ability to navigate and interact with the healthy care system independently
- Support the Healthy Choices Diabetes program to engage in literature reviews and research as directed by Healing and Wellness Managers
- Develop a streamlined referral network for diabetes related issues
- Support MNO Healing and Wellness Management as required in the spending/reporting of discretionary funding
- Monitor outcomes of the program through reporting requirements
- Participation in MNO Healing and Wellness training as needed/determined.
- Participate in on-going professional development activities as time and financial resources permit
- Attend training and meetings outside Métis Nation of Ontario working hours
- Fulfill all other responsibilities as determined by management of Healing and Wellness Branch
- Financial reporting specific to your program site and budget
- Maintain confidentiality at all times, and adhere to privacy legislation
**Qualifications**:Mandatory qualifications**:
- Experience working with volunteers in a non-profit environment
- 1 or more years’ experience in program development
- Proficiency with computer programs (Outlook, Excel, Word and PowerPoint)
Preferred
- Degree or diploma in social services preferred
- Experience working directly with clients
