**1 ‑ Temporary Full Time - Nurse Practitioner**
**Palliative Care Clinical Coach & Outreach Nurse Led Long‑Term Care Outreach**

The individual will report to the Manager of the Nurse Led Outreach Team (NLOT), with a relationship with the
Palliative Care physician's group to develop and establish the role of Palliative Care Transition and Outreach Nurse
Practitioner. The Palliative Care Clinical Coach and Outreach Nurse Practitioner will be an individual with an
understanding of palliative care, including complex symptom management and illness trajectory, as well as
understanding the complexities of system navigation in the city of Thunder Bay and the region served by TBRHSC.
The position is responsible for working with participating community organizations in alignment with provincial
direction established by Ontario Health and guidance developed by the Ontario Palliative Care Network. The
Palliative Care Clinical Coach and Outreach Nurse Practitioner will drive practice change, with a focus on improving
access to and quality of clinical care, aligned to the Palliative Care Health Services Delivery Framework in the
Community and the Ontario Palliative Care Competency Framework.
EMPLOYMENT REQUIREMENTS:
Education/Experience
*Current registration as a Registered Nurse in the Extended Class with the College of Nurses of Ontario
*Successful completion of a Master's Degree preferred
*Current Basic Cardiac Life Support (BCLS) certificate is required
*Active Professional Liability Coverage required
*Membership with RNAO or NPAO is preferred
*Minimum of 3 years recent nurse practitioner experience during the last 5 years is preferred.
*Advanced training in palliative care preferred, e.g. Learning Essential Approaches to Palliative Care, Hospice
Palliative Care Nursing Certificate, or equivalents.
*Experience in the development, implementation and evaluation of relevant advanced nursing practices is required.
*Experience in the development of strategies to build capacity and improve health outcomes is required.
*Experience in the development of clinical guidelines and protocols, promoting the use of research and evidence
based practice to guide patient centered care is required.

all people in Ontario, regardless of their age or illness.
*A good understanding of regional priorities and local health care needs, within Ontario's health care landscape.

*Aptitude for proactive problem solving, using strong critical thinking, and negotiating skills.
*A demonstrated ability to develop, deliver and evaluate training and coaching efforts.
*An ability to develop relationships based on trust, compassion, and empathy.
*Strong active listening and interpersonal communication skills for one‑on‑one coaching.
*Experience designing education to effectively address palliative care training needs of inter‑professional learners.
*Excellent communication skills and experience with culturally responsive teaching
*A demonstrated focus on person‑centered care.
*Supports the advancement of equity, inclusion, and diversity across Ontario's health care system.
*Commitment to engage with First Nations, Indigenous, Métis and urban Indigenous people as they self‑determine
their own approach to palliative care.
*Proficiency in educational technologies (e.g., webinars, apps) as well as knowledge of Microsoft Outlook, Word,
Teams, PowerPoint, and Excel.
*Access to reliable and efficient mode(s) of transportation to enable travel between community organizations. Effort‑

Salary Range: $65.210 to 70.750
