**MENTAL HEALTH OUTPATIENT UNIT**

**BEHAVIOURAL SUPPORTS ONTARIO TRANSITIONAL FACILITATOR & TEAM LEAD**

:

- Coordinate clinical activity with the team;
- Act as a resource to the inter-professional team in the provision of safe and therapeutic patient care;
- Promote optimal allocation of resources utilizing process optimization;
- Work with team members to ensure standards of care are met through role modeling, auditing and consultation;
- Support quality care initiatives, team education and coaching as appropriate
- Assist the Mental Health manager in the development and promotion of new policies, best practices, standard operating procedures and on-boarding material as it relates to the BEHAVIOURAL SUPPORTS ONTARIO population and model;
- Promote and maintain collaborative team relationships internally and externally to the Hospital
- Support and foster system-wide patient care planning and critical thinking

**HOURS**:
Days, Monday through Friday 0800-1600 hours. In addition, this position must be flexible to occasional changes in hours when required to meet the operational needs of the program.

**SKILLS & QUALIFICATIONS REQUIRED**:

- Current Registration with the College of Nurses of Ontario in good standing;
- Minimum five (5) years’ experience working in an Acute Care setting with a geriatric population required;
- Minimum of three (3) years’ experience working in Long-term Care settings required
- Minimum of three (3) years’ experience working in a leadership capacity such as Unit Lead, Clinical Resource Nurse required
- Those with team lead, supervisor, or manager experience will be given preference
- Specialized skills in the assessment of delirium, dementia, and mental health with knowledge of best practices in this area;
- Five (5) years’ experience demonstrating excellence crisis intervention skills, and the assessment, treatment, and development of care plans for complex geriatric mental health patients;
- Strong working knowledge of psychopharmacology and medication side effects;
- I.E.C.E.S. ((Physical, Intellectual, Emotional Capabilities, Environment and Social) & GPA (Gentle Persuasive Approach), training, required; Montessori training certification, preferred;
- Strong working knowledge of DSM-IV-TR, the Mental Health Act, and PHIPA (Personal Health Information Protection Act);
- Excellent interpersonal and team building skills that enable rapport building with stakeholders;
- Proven ability to provide coaching and mentoring;
- Proven ability to deliver under tight deadlines with effective time management skills;
- Proven leadership skills that foster engagement and strength based outcomes;
- Self-starter who can work with limited guidance;
- Well-developed analytical and problem-solving skills;
- Excellent knowledge of community resource within Elgin County and surrounding area;
- Personal transportation with a reliable vehicle and holds a valid Ontario Class G driver’s license and willingness to travel, required;
- Demonstrated exceptional organizational, interpersonal and communications skills (both written and oral);
- Demonstrated knowledge and commitment to patient and staff safety;
- Demonstrated acceptable previous performance reflective of STEGH's Mission, Vision, Values and Code of Conduct;
- Demonstrated ability to attend work on a regular basis in accordance with STEGH’s Attendance at Work Program.
