Patient Flow Manager

The Patient Flow Manager will oversee and facilitate effective and efficient patient admissions, transfers, and discharges for the non-forensic program, supporting CAMH patient flow guidelines and processes. This role will involve developing and maintaining positive relationships with internal and external stakeholders to ensure seamless access across the continuum of care, overseeing data collection for resource utilization analysis, and collaborating on operational processes.
**Key Responsibilities**
- ** Patient Flow Management**
- Collaborate with team leads, charge nurses, and multidisciplinary teams, including physicians and administrative staff, to ensure timely and safe admissions, transfers, and discharges.
- Monitor and manage ED volume and direct patient flow between departments and external hospitals.
- Follow up with inpatient teams on bed capacity, discharge forecasts, and transfer needs.
- Plan and coordinate resources for incoming patient admissions and transfers.
- Participate in daily system capacity calls with ~27 partnering hospitals to balance regional bed supply and demand.
- Develop emergency response strategies for patient surges and coordinate effective utilization of resources.
- ** Strategic Care Planning**
- Forecast bed capacity and conduct strategic care planning to match patients to appropriate beds, considering health equity and operational challenges.
- Review patient charts and collaborate with internal and external teams for care coordination.
- Facilitate clinical discussions to align on patient care needs and admission/transfer priorities.
- ** Communication and Coordination**
- Ensure timely communication and coordination across departments for seamless patient care.
- Develop and maintain relationships with external healthcare providers for continuity of care.
- Provide regular updates to hospital leadership and stakeholders on patient flow metrics and challenges.
- ** Quality and Process Improvement**
- Identify and address inefficiencies and implement quality improvement initiatives.
- Monitor and maintain quality and safety standards in patient flow processes.
- Analyze data to optimize patient flow operations and inform process enhancements.
- ** General Responsibilities**
- Participate in risk management, patient safety, and quality improvement activities.
- Maintain compliance with health and safety regulations, professional standards, and hospital policies.
- Prepare reports for clinical leadership and stakeholders.
- Model principles of integrity, responsibility, and respect.
- ** Emergency Preparedness**
- Develop emergency response plans for surges and coordinate with team leads for effective resource use.
- Participate in emergency preparedness drills.
- ** Resource Allocation**
- Work with program directors and inpatient managers to ensure optimal staffing levels.
- Ensure efficient use of facilities and equipment to support patient flow.
**Job Requirements**
- Master's degree in a related field (e.g., Nursing, Social Work, Occupational Therapy, Health Sciences Administration)
- Registered member in good standing with the relevant regulatory college
- Minimum of 5 years in a healthcare setting, focusing on mental health care and management.
- Prior supervisory or leadership experience is highly beneficial.
- Demonstrated knowledge of mental health, addiction services, and bed management systems.
- Strong interpersonal and communication skills with proven success in collaboration.
- Experience with client

- Proficiency in Microsoft Office Suite and Adobe.
