Are you a dynamic, customer service‑oriented individual who is eager to use your clinical administration skills to contribute to the high‑quality care that SickKids provides? If so, then we have an opportunity for you The ABILITY program supports neuro‑divergent children/youth accessing tertiary level care at SickKids hospital, including in‑patient, emergency department visits and out‑patient clinic visits. As a Patient Flow Coordinator, you will work with a highly specialized interdisciplinary team providing high quality patient care and coordination within the ABILITY program. You will enhance and maintain the program’s operations by coordinating and facilitating the efficient flow of patients through all stages of their visit. You will support all processes involved in a visit from registration to booking follow‑up appointments, direct patient care tasks such as vitals and flow management to being the primary coordinator for adaptive care planning visits which enable neuro‑diverse patients in accessing timely planned visits to SickKids. This position involves being in clinic 1 day/week, and organizing, prioritising and collaborating with other teams to ensure seamless care is coordinated for the other 4 days/week. Here’s What You’ll Get To Do In Clinic (1 day/week): Responsible for the efficiency of all stages of the patient’s clinic visit, facilitating daily clinic, including greeting patients, taking measurements and recording in patient chart, managing rooming, preparing exam rooms, and assisting with physical exams when appropriate Complete simple discharge instructions, complete handover meetings post clinic Answer phone calls and emails – respond to requests for information from families and healthcare staff Book all clinic appointments and follow‑up appointments Reschedule appointments as necessary and inform the team of any changes Maintain appointment schedules on the computer system and notify patients/families of any updates or changes to their appointments Assist with triaging of external referrals (prepare spreadsheet for weekly triage meeting). Complete any follow‑up of referrals as discussed with triage MD lead Strong critical thinking, written and oral communication skills required Strong organisational and data management skills essential Main care coordinator for ABILITY Adaptive Care Plans (3 clinics/week) – supporting patients requiring individualized care planning for tertiary level care which entails planning from home to hospital back home again Collate clinical data and care plans and relay this information in a precise and timely manner to colleagues within and outside the organization Provides superior service excellence to all colleagues, including patients and families, and internal and external partners Here’s What You’ll Need Minimum of 3 years clerical or administrative experience in a health care setting. Experience in a hospital environment is preferred Bachelor’s Degree in a related field (Medical Administration, Business Administration, Health Administration) preferred and/or Diploma from a recognised Administrative/Secretarial programme or equivalent work experience. Medical terminology is an asset Scheduling experience is essential Accurate keyboarding skills and strong working knowledge of Word, Excel, PowerPoint, and Outlook. Strong navigation of EPIC is an asset in this position Excellent problem‑solving skills that demonstrate an ability to anticipate potential issues and make suggestionsStrong interpersonal, communication, and organisational skills with both fellow colleagues and patients Ability to remain focused and prioritise tasks in a busy and high‑stress environment Excellent prioritisation and time‑management skills Demonstrated commitment and actions in advancing equity, diversity, and inclusion objectives Here’s What You’ll Love A focus on employee wellness with our new Staff Health and Well‑being Strategy.
