**The Opportunity**:
Under the direction of the Manager, the Clinical Intake Counsellor is responsible for ensuring that all referrals received for Mental Health Services are appropriately collected and processed through Outpatient Mental Health’s intake procedures. The Clinical Intake Counsellor is also responsible to ensure that referrals are made to the most appropriate service to support clients most effectively and least intrusively. Additionally, The Clinical Intake Counsellor will ensure all required documentation duties are fulfilled as per policy. The Clinical Intake Counsellor functions within legislative requirements, regulations, policies, procedures, and the I CARE AT values of Collingwood General and Marine Hospital

**Qualifications**:

- Currently registered in good standing with a health regulatory College in the Province of Ontario.
- Has two (2) years’ professional experience in a Healthcare or Social Service environment.
- Possesses comprehensive knowledge of the Mental Health Act, Health Care Consent Act and Substitute Decision Maker Act, as well as Mental Health screening and intervention methods.
- Knowledge of external Mental Health services providers and resources available in the South Georgian Bay region would be considered an asset.
- Demonstrated crisis intervention, conflict resolution, mediation, and problem solving capability.
- Must strive to continuously provide a safe and supportive environment for patients, staff, physicians, and volunteers
- Must adhere to the duties of workers, as stipulated in the Occupational Health and Safety Act
- Must have an ability to effectively communicate (written and verbal) in the English language in order to provide excellent communication with team members, patients and families. Ability to communicate in additional language(s) is considered an asset.

- Must be able to perform all bona fide and essential duties of the position, which may include physical demands such as moving/transferring patients, pushing, pulling, bending, etc.
- Must have demonstrated excellent attendance/performance record

- Ability to work independently and effectively communicate with other members of the health care team

**Duties and Responsibilities**:

- Receives new referrals and meets with patient/families in-person as scheduled to conduct initial program intake and identifies suitability for connection to community services and resources.
- Completes clinical intake interview either in-person or over the phone as required.
- Completes detailed information including: presenting concerns, family history, school and education information, social functioning and strength and needs of the patient/family.
- Creates a service plan with the patient and makes all necessary service recommendations and referrals as indicated, both internally and externally.
- Completes appropriate screening questionnaires with patients as they relate to presenting concerns.
- Discusses policy practice regarding patient consent, confidentiality practices, circle of care procedures and internal complaint procedures.
- Discusses and documents patient consent using appropriate forms.
- Assesses mental health risk utilizing risk and assessment tools, standards, guidelines and policies and procedures.
- Supports patients presenting as in crisis and respond as per policy and clinical guidelines.
