Jean Tweed is a leading, community-based organization that provides a supportive environment for women with substance use, mental health, and gambling concerns. Recognizing the diversity of women’s experience in today’s society, our programs seek to address a range of issues in a woman’s life that may contribute to substance use and gambling concerns.

This role provides counselling services to Indigenous individuals, groups, and family members for substance use and related concerns as required. Liaises with other professionals to create culturally-specific treatment plans for a broad range of substance use issues. Coordinates support and outreach activities for Indigenous individuals that may include: coordination of culturally-specific substance use support and related services, motivational counselling, brief assessment and referral, conducting education and information services, monitoring and supporting daily activities of clients.
**How will you help to get us there?**
As an Indigenous Case Manager, you will:
- Establish a comprehensive, holistic, individualized service plan with each client on caseload.
- Provide case management support including counselling, outreach, on-going assessment, goal planning, referral, development of support systems, system navigation, etc.
- Provide referrals and support to access health and community-based services.
- Support activities geared towards establishing and maintaining healthy lifestyles with emphasis on primary health care, mental health, relapse prevention, community engagement, capacity-building, conflict resolution, crisis management and life skills.
- Develop and maintain relationships with other service providers and community resources.
- Participate in the on-call rotation as required.
- Complete required case notes, evaluations, and data entry reports in an accurate and timely manner.
- Collaborate with supervisor to discuss job performance and establish professional goals.
- Engage in ongoing staff development and personal learning as agreed to in performance reviews.
- Conduct job responsibilities with a commitment to harm reduction, trauma
- informed, women-centred and anti-oppressive practices.
- Abide by the Occupational Health and Safety Act, and work in a manner that is safe, reporting incidents immediately to direct supervisor.
- Adhere to the policies and procedures of The Jean Tweed Centre.
- Perform other duties as assigned in order to meet the overall goals and objectives of The Jean Tweed Centre.
- Maintain a client service orientation including learning about and addressing client needs and concerns.
- Work collaboratively including respect others, sharing information, consulting and celebrating shared successes.
- Demonstrate accountability by holding self-responsible and following through on commitments.
- Support inclusion and equity by respecting others, embracing diversity and addressing oppression.
This role reports to the Manager of Housing.
**What will you bring?**
- Lived experience within Indigenous community/culture including familiarity and knowledge of Indigenous traditions, ceremonies and values.
- Bachelor’s Degree in Social Work, or a related discipline.
- A combination of 3 years of work experience in the field of Mental Health, Substance Use, and/or related field.
- Expertise in providing counselling and case management to individuals who are experiencing or have experienced a complex history of mental health concerns, substance use, and/or homelessness.
- Working knowledge of best practices in mental health, substance use, harm reduction, trauma and the therapeutic relationship.
- Working knowledge of psycho-social rehabilitation and recovery principles.
- Working knowledge of trauma-informed practice, women centred principles and best practices for women and substance use.
- Working knowledge of housing, health care and community resources in the Toronto area and related provincial resources.
- Ability to work collaboratively in complex community settings.
- Ability to use a range of technological tools, e.g. online reporting systems, videoconferencing etc.
- Ability to work independently with mínimal day to day supervision.
- Crisis intervention skills, with well-developed problem-solving abilities and interpersonal skills.
- Excellent oral and writing skills.
