Case Manager- Transition Homes

Position Summary:
Wasauksing First Nation is seeking a dynamic Transitional Housing Case Manager. Reporting to the Shelter Manager, the Case Manager ensures a safe and welcoming environment for transitioning Emergency Safe Home residents to Transitional Housing. Our mission is to provide temporary transitional housing, while providing outreach, education services and housing support for Indigenous women, men, families and 2SLGBTQIA+ individuals who are victims of domestic violence and intimate partner violence.
Job Duties & Responsibilities:
- Complete intakes, assessments and personal case plans;
- Implementing, updating and reviewing ongoing individual case plans;
- Determining the appropriate services, completing referrals and engaging in necessary follow‑up to meet clients' needs;
- Collaborating with appropriate community service partners as part of an interdisciplinary case management team to meet the goals of the personal case plans;
- Coordinating recreational and psychoeducational activities;
- Maintaining accurate records of all encounters with clients and case management, all while maintaining client privacy & confidentiality;
- Developing and maintaining a 16‑week health promotion program;
- Coordinating outreach activities for residential and non‑residential clients, and follow‑up support in the community;
- Maintaining a warm, welcoming, respectful, and non‑judgmental environment for staff and clients;
- Maintaining a strong understanding of policies and procedures for the Shelter and Transition Homes;
- Establishing and proactively maintaining healthy with all staff;
- Keeping up to date documentation, reports, routine repairs & inspections and preventative measures in place for equipment, furniture, electrical, and appliances of transitional homes;
- Building strong knowledge and procedures for routine maintenance for transitional homes for client and staff safety;
- Sharing information regarding updated/relevant, and appropriate community services to benefit shelter and transition home residents;
- Providing relevant and appropriate advocacy and support to program participants while abiding by Transition Home policies and the employee Code of Conduct;
- Establishing and building working relationships with external partners;
- Participating in training and weekly staff meetings;
- Organizing, providing a clean workspace and assisting with cleaning duties;
- Adhere to all policies and procedures of Wasauksing First Nation and any other relevant policies and procedures from external agencies and partners;
- Perform other duties within the scope of the position as assigned by the Manager Or Supervisor;
Education & Experience:
- A degree in social work/counselling psychology, social service work diploma or an equivalent combination of education, training, and experience is considered an asset.
- Minimum 2-3 years’ experience working in the front line in social service delivery is an asset.
- Valid First Aid & CPR is required.
- Non‑Violent Crisis Intervention, Suicide Prevention and Intervention and Trauma Informed Care training are considered assets.
- Training will be provided to the selected candidate.
Other Requirements:
- Work in compliance with the OH&S Act and Regulations.
- Significant understanding and knowledge of systemic barriers and oppression, its roots, and how they affect First Nations.
- Knowledge of surrounding community supports and services.
- Knowledge of (but not limited to) Mental Health Act, Canadian Human Rights Act, Child and Family Services Act, Housing Services Act, Bill 173, etc.
- Demonstrated ability to work independently and as a team.
- Effective communication and interpersonal skills.
- Demonstrated knowledge of, or commitment to learn, culture, protocols, traditions, and ideology of Indigenous Peoples and organizations in the area.
- Demonstrated knowledge of Indigenous worldviews and perspectives of health, wellness and traditions of caring (considered an asset or required).
- Excellent understanding and experience in creating and maintaining boundaries.
- Provide a current and cleared Vulnerable Sector Check, CPIC.
- Valid Driver's License and reliable personal vehicle.
- Willing to work flexible hours as required, including evenings and weekends.
