**Primary Responsibilities**:
Under the direction of the Supervisor of Holistic Healing Team 5, the Holistic Case Manager will:

- Work alongside coworkers, including Child and Family Wellbeing workers to ensure that caregivers and families are provided with required support.
- Be knowledgeable about child welfare practices and relevant legislation to assist families in understanding their responsibilities and rights.
- Deliver all services with care and kindness; maintaining professionalism consistent with NCFST’s established standards, policies and procedures.
- Maintain good working relationships with community partners.
- Conduct holistic assessments and co-create goals/objectives with community members; identify appropriate resources (external and internal to NCFST) to meet needs.
- Complete referrals to other services.
- Provide ongoing support and advocacy to ensure quality of service to community members through collaboration with all service providers (in accordance with current privacy legislation).
- Provide supportive counselling and practical supports as required. This may include home visits, community visits or accompaniments to other organizations in addition to visits that take place in office.
- Gather collateral information and work with other service providers (internal and external to NCFST) in accordance with privacy legislation.
- Attend and/or facilitate circles to coordinate care between service providers.
- Complete all required documentation including ongoing record of service (casenotes), statistical reporting and input into NCFST’s computerized case management system.
- Identify and refer cases to the appropriate resources and supports.
- Work in a safe manner in accordance with the Society’s Health and Safety Policies and Procedures and all relevant legislation
- Must be comfortable using Microsoft software including Outlook and Teams.
- Communicates effectively both orally and written.
- Other duties as required.

**QUALIFICATIONS, KNOWLEDGE, AND SKILLS**:

- Must have experience working with Indigenous communities; preference will be given to those who identify with an Indigenous community
- Post Secondary education or training in a relevant field and experience working with Indigenous Peoples
- Pass a vulnerable sector police records check
- Knowledge and awareness of the diverse urban Indigenous population in Tkaronto
- Understanding of First Nation, Metis and Inuit community-based work.
- Team skills to work collaboratively with others
- Effective counselling and crisis support skills
- Excellent communication skills, both oral and written
- Time management, organizational and coordination skills
- Understanding of Native Child and Family Services of Toronto Mission and values
- Understanding of CYFSA and other legislation related to child welfare
- Understanding of workplace Health and Safety practices and understanding of an employee’s responsibility under current legislation
- Ability to work effectively with all levels of staff, to maintain effective communication and working relationships, demonstrating strong interpersonal skills, tact, sensitivity and build strong internal relationships
