Join to apply for the Thrive Case Manager role at Covenant House Toronto Requisition Number: CMS-2025-002 Overview The Case Manager, Thrive Newcomer Program is responsible for developing and overseeing case management for youth assigned to their case management team and for providing case management leadership and mentoring to the team. This position requires a high level of engagement with diverse populations, including youth who are newcomers, refugees, and those with precarious immigration status. A strong working knowledge of immigration systems, legal supports, settlement services, and barriers faced by newcomer youth is essential. The role involves relationship building, assessment, collaborative planning, monitoring/evaluation, advocacy, and systems navigation support involving internal and external services and resources. Responsibilities Provide ongoing case management engagement for youth assigned to the Case Manager and their team, with an emphasis on inclusive, culturally competent support for newcomer and immigrant youth. Collaborate with youth to assess their needs, discuss options, and develop a case plan that reflects their strengths and lived experiences with sensitivity to cultural identity, migration history, and legal status. Assess risks and develop safety plans in collaboration with youth and coordinate proactive risk mitigation strategies with the Thrive team. Provide mentorship and oversight to team members in implementing individualized case plans. Maintain up-to-date, accurate, and professional case documentation that aligns with the standards and ethical guidelines set out by the Ontario College of Social Workers and Social Service Workers (OCSWSSW), including timely recording of assessments, case plans, progress notes, risk evaluations, service referrals, and case reviews. Ensure all documentation reflects a client-centered, strengths-based approach, respects client confidentiality and informed consent, and upholds principles of transparency and accountability, and professional integrity. Ensure case management includes appropriate referrals to settlement services, immigration legal supports, language access resources, and trauma-specific services for those impacted by forced migration or conflict. Facilitate access to internal and external service providers, including supports related to physical/mental health, substance use, housing, and immigration pathways (e.g., legal aid, refugee/asylum claims, PR applications). Coordinate case conferences and consultations, advocating for youth within systems such as IRCC, Legal Aid Ontario, and settlement agencies. Maintain up-to-date knowledge of community resources, including newcomer and immigration-specific supports. Participate actively in program development, interdepartmental collaboration, supervision, and continuous learning. Contribute to quality improvement initiatives and evidence-based practice within the department. Other additional duties as assigned. Qualifications BSW or MSW degree, with 5 years of related experience, including at least 2 years working with youth experiencing homelessness. Registration in good standing with the College of Social Workers and Social Service Workers. Experience working with newcomer, refugee, and immigrant youth is strongly preferred. Demonstrated understanding of immigration systems, legal protections, and barriers affecting youth with precarious status (e.g., human trafficking, unaccompanied minors, refugee claims, family reunification issues). Strong commitment to an integrated, anti-oppressive, trauma-informed, harm reduction, and housing-first approach. Proven ability to manage complex cases involving housing instability, substance use, mental health, and intersectional identities, including immigration-related vulnerabilities. Excellent interpersonal, advocacy, and communication skills. Familiarity with documentation practices, outcome measurement, and reflective supervision. What we Offer Meaningful work.
