**JOB DESCRIPTION**

**PROGRAMS & COMMUNITY OUTREACH COORDINATOR**

**REPORTS TO**: Executive Director

The **Programs & Community Outreach Coordinator** is responsible for supporting clients and providing leadership in the coordination of day-to-day programs. This role involves building relationships with local community groups and organizations to increase awareness of Hospice Georgina’s services while ensuring programs meet community needs and maintain the highest standards of care.

**DUTIES AND RESPONSIBILITIES**

**Program Coordination and Client Support**:

- Ensure the provision of cost-effective client services and programs.
- Assess clients for service needs, monitor service delivery, and contribute to the development, evaluation, and promotion of programs.
- Develop and implement program plans to meet organizational goals and community needs.
- Conduct regular client engagement and follow-ups to ensure programs meet expectations.
- Ensure all client forms, including Service Agreements and consents, are completed and maintained.
- Process referrals professionally, ensuring clients are redirected to appropriate resources when necessary.

**Volunteer Management**:

- Provide ongoing support and resources to volunteers.
- Maintain clear communication with volunteers, addressing concerns or program-related issues promptly.
- Collaborate with the Executive Director to address volunteer concerns or complaints as needed.

**Community Outreach**:

- Build and nurture relationships with local organizations and community groups to raise awareness of Hospice Georgina’s services.
- Represent Hospice Georgina at community events, workshops, and meetings to strengthen partnerships.

**Administrative and Reporting**:

- Maintain accurate records for clients, volunteers, and program activities in compliance with organizational policies.
- Support financial accountability by maintaining program budgets.
- Prepare required reports for the Executive Director.
- Submit a summary of coordination activities for monthly Board meetings and prepare an annual report for the AGM.

**Compliance and Confidentiality**:

- Ensure client and volunteer files are securely stored according to organizational policies.
- Maintain strict confidentiality and adhere to Hospice Georgina’s standards at all times.

**SKILLS & QUALIFICATIONS**
- Bachelor’s degree in Social Work, Health Sciences, Community Development, or related field (or equivalent experience).
- Experience in program coordination and development, community outreach, and/or volunteer management.
- Experience in the non-profit sector.
- Strong organizational and time-management skills, with the ability to manage multiple priorities.
- Excellent interpersonal and communication skills, both verbal and written.
- Proficiency in Microsoft Office Suite and client/volunteer management software.
- Experience with budget management and report writing is an asset.
- Knowledge of community resources and networks within the Georgina area is preferred.
- Valid Driver’s License: A valid driver’s license and access to a reliable vehicle are required for travel within the community.

**ONBOARDING AND PROFESSIONAL DEVELOPMENT**

New hires will participate in an onboarding program that includes:

- Orientation to Hospice Georgina’s mission, values, and policies.
- Mandatory Hospice Palliative Care of Ontario training
- Training on client and volunteer management systems.
- Introduction to key community partners and resources.

Ongoing professional development opportunities will be available, including workshops, webinars, and access to relevant certifications.

**HOURS OF WORK**
- Monday to Thursday: 8:30 AM - 4:30 PM
- Occasional evening and weekend availability required for community events or program needs.

Pay: $25.00 per hour

Expected hours: 32 per week
