Reporting to: Manager, Provincial Resource Centre - Organ
- Classification: Provincial Resource Centre Flow Navigator (full time)
- Worksite: based out of 482 Bay Street
- Schedule: Combination of weekday and weekend 12 hour night shifts (7pm -7 am)
- Number of positions: 2: 1 permanent full time and 1 permanent part time (0.50)
- Trillium Gift of Life Network (TGLN), part of Ontario Health, is responsible for the coordination of organ and tissue donation and transplantation in Ontario. Our mandate includes increasing public awareness of the importance of organ and tissue donation, education of health care professionals, and ensuring a safe and effective donation and transplant system in Ontario. Working collaboratively with our stakeholders and fostering organizational and individual growth, we value honesty, trust, respect and compassion.
- The Provincial Resource Centre (PRC) Flow Navigator is an integral member of the PRC Team and the provincial allocation and recovery system. The Flow Navigator is responsible for smooth and efficient patient case flow within the PRC after hours and on statutory holidays. The Flow Navigator is the designated team lead after hours, on weekends, and on statutory holidays and is the first point of contact for all PRC staff. The Flow Navigator is responsible for addressing any and all operational issues that arise in consultation with the Manager-on-Call. Responsibilities include the oversight of operations in the PRC after hours and on statutory holidays, coordination of patient flow, liaising with internal and external partners as needed, providing support to staff, and delegating case assignments to ensure safe, effective, quality patient care as it relates to organ allocation and recovery.
- Key areas of responsibility include:
- Facilitating patient flow throughout the provincial donation/transplant system- Coordinating and organizing CSC, RTC, and SRC resources- Stakeholder relationship management- Supporting a safe and healthy work environment for the PRC Team- Acting as a resource to staff, Transplant Support Physicians, Transplant Hospital Programs, ORNGE Air Ambulance, and other stakeholders
- Here is what you will be doing:
- Work collaboratively with the Manager-on-Call to effectively and safely utilize resources for quality patient case management and flow.- Be easily accessible to staff to respond to requests for assistance and guidance when staff encounter complex situations that require expert support.- Understand the process for organ referral, allocation, and recovery in Ontario.- Experience with using OATS is preferred.- Proactively identify issues and/or barriers to safe, quality patient case management and flow within the PRC and work with the Manager-on-Call to safely resolve the issues.- Provide leadership with after-hours operational decision-making.- Be knowledgeable about the Collective Agreement that governs the roles and responsibilities of staff in the PRC.- Effectively coordinate CSC, RTC, and SRC resources according to patient flow needs.- Facilitate regular communication with all teams ensuring optimal patient flow and case management.- Lead bullet rounds during weekends and statutory holidays.- Help build capacity in staff members through mentorship, interpretation of policy, and facilitating communication between partners.- Participate in activities such as Joint Coordinator meetings, quality of care reviews, and other meetings that support overall organizational performance improvement.- Complete both the RTC and CSC shift reports and distribute them a
