about the role:
Sherbourne Health has been funding by Immigration, Refugee and Citizenship Canada (IRCC) to provide our Finding Health Project that supports newcomers, immigrants and refugees as they adapt to living in Canada. Our project is dedicated to delivering services at the intersection of health and settlement.
The System Navigator (Well-Being Navigator) helps newcomer clients navigate to resources and information on the Canadian health and mental health care system, with a priority placed on supporting eligible clients with complex or higher-needs. This includes identifying available resources, facilitating access to care, and strengthening connections between health, social service, and settlement organizations. The role is essential for filling service gaps, especially for clients who are waiting for case management or whose needs exceed current service capacities.
The System Navigator (Well-Being Navigator) collaborates closely with clients, internal clinical teams, community partners, and external agencies to ensure holistic, coordinated care.

Responsibilities and Tasks:
Case Management & Navigation Services
- Deliver trauma-informed case management and system navigation support to newcomer clients with complex needs, including those experiencing homelessness, mental health concerns, or substance use, in a responsive and flexible manner that complements and augments existing support for newcomers, immigrants and refugees
- Conduct client assessments and develop care plans in collaboration with internal teams and external partners
- Provide crisis intervention, advocacy, and systems navigation particularly for high needs and complex clients that require additional support and/or do not qualify for other services
- Facilitate referrals and ensure timely access to health and social services
- Participate in care planning and case coordination meetings
- Document all client interactions using Accuro EMR and required data reporting systems
Well-Being Navigation Activities
- Help clients access and understand provincial/territorial health coverage and programs such as the Interim Federal Health Program
- Provide information about the Canadian health and mental health systems, including provider roles and service options
- Refer clients to appropriate health, social, and settlement services
- Prepare clients for health-related appointments and care transitions
- Build and maintain partnerships with hospitals, community health agencies, and Local Immigration Partnerships (LIPs)
- As capacity allows, support the delivery of group education sessions or webinars on health and wellness topics
Health Promotion, Education and Advocacy
- Advocate for clients to ensure access to appropriate care and resources
- Educate peers and community members on client-related health topics
- Engage in community outreach to build relationships with partner agencies
- Support systemic advocacy initiatives to address health inequities and the social determinants of health
- Promote culturally safe and equity-focused practices throughout all work
Administrative Duties
- Maintain accurate and timely client records
- Contribute to program planning, evaluation, and development
- Operate within organizational policies and procedures
- Participate in internal meetings, training, and initiatives
- Other duties as required
To thrive in this role, you'll need
education/experience:
- Bachelor's degree in Social Work or a related health or social service discipline
- Minimum 2 years of case management or system navigation experience is an asset
- Minimum 3–5 years of experience in community health and/or settlement sectors
- Experience providing outreach and client support in a community setting
- Demonstrated ability to work collaboratively in a multidisciplinary environment
- Experience with crisis de-escalation, safety planning, and trauma-informed practices
- Proficiency in EMR systems (ideally Accuro) and Microsoft Office tools
- Registration with the Ontario College of Social Workers and Social Service Workers is an asset
skills and abilities:
- Thorough knowledge and experience working with diverse newcomer and refugee populations, including LGBTQ newcomers, racialized newcomers and/or newcomers experiencing homelessness or precarious housing
- Experience with harm reduction, trauma-informed approaches, addictions and mental health and issues
- Knowledge of criminal justice system, Ontario Works, and the Ontario Disability Support Program
- Skilled in navigating complex systems and advocating for vulnerable populations
- Excellent oral and written communication skills; strong computer skills and experience using database software.
- Ability to carry out duties independently while maintaining accountability to agency.
- Fluency in a French and/or another second language is an asset

working conditions:
- Ability to work a fixed schedule and have flexibility to work evenings and weekends.
- This position will work on site at Sherbourne Health and in the community with clients where needed.
- Must be able to work with clients with varying levels of complexity, including individuals with intersecting health, mental health, and social challenges.
