Indigenous Outreach and Engagement Coordinator, Ft

Niagara Health is located on the traditional territory of the Haudenosaunee and Anishinaabeg peoples. This territory is covered by the Upper Canada Treaties and is within the Dish with One Spoon Wampum agreement. In the spirit of respect and friendship Niagara Health is embarking on a new quality improvement collaborative to better support Indigenous patients and their families.
Reporting to Manager, Indigenous Health and Reconciliation and working collaboratively with the Indigenous Health and Reconciliation Team and other members of Niagara Health, the Indigenous Outreach and Engagement Coordinator will be vital to the building of community relationships through the engagement, education and resource coordination.
RESPONSIBILITIES:
- The role will have a focus on the following areas of service delivery:
- Provide support for Indigenous Patients Relations Specialists,
- Maintain a high level of confidentiality and safety in support of a respectful, inclusive and compassionate care journey for Indigenous patients, their families and caregivers, and members of their care team.
- Document patient stories where appropriate.
- Foster an environment of cultural safety for all patients, caregivers, and their families;
- Build relationships with diverse traditional practitioners in order to facilitate access to traditional healing practices including, cultural and spiritual practices for patients and families at their request.
- Provide support to external practitioners to ensure Niagara Health policy and procedures are understood and adhered to;
- Work collaboratively with local Indigenous communities to identify opportunities and coordinate educational sessions to improve health outcomes for Indigenous Peoples
- Coordinate and promote engagement events to identify and prioritize improvement opportunities to support Indigenous health
- Develop outreach strategy, workshops, presentations, and experiences related to health care specific to Indigenous communities
- Engage with community organization, staff to promote programming information
- Support local Indigenous individuals to engage in activities aimed at improving the health and wellbeing of community/Indigenous Health providers
- Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
- Work Collaboratively with all members of the Niagara Health community
- Work collaboratively with the Human Resources department to participate in recruitment activities as needed
- Additional daily activities and multi-site travel may be required. The incumbent may be required to support activities outside of regular business hours.
**QUALIFICATIONS**:
- Five years of navigation, coordination, or other similar experience in healthcare or the social services sectors;
- Lived experience (must self-identify as Indigenous);
- Strong knowledge of the history of Indigenous Peoples and the impact of intergenerational trauma; Trauma informed
- Knowledge of local Indigenous organizations and community resources;
- Knowledge and experience with traditional ceremonies and healing practices;
- Ability to perform some culturally relevant ceremonies would be an asset;
- Excellent communication skills (both written and oral);
- Superior interpersonal, collaboration and facilitation skills;
- Ability to facilitate workshops and presentations to diverse groups
- Strong problem-solving abilities;
- Demonstrated ability to work effectively in a multi-disciplinary team environment;
- Ability to travel across the region, valid driver's license and access to a reliable vehicle is preferred
- Consistent and regular attendance required
- Ability to speak an Indigenous language is an asset.
- Vaccines (COVID-19 and others) are a requirement of the job unless you have an exemption based on medical or on a ground pursuant to the Ontario Human Rights Code.
