Intake Facilitator (25-0000044)

Department: Mental Health Outpatient Program
Site: St. Joseph's Health Centre
Location: Thunder Bay, Ontario
Shifts: Days
**Rate of Pay: $41.17 - $49.77per hour**

Reporting to the Clinical Manager - Mental Health Outpatient Program (MHOP), the Intake Facilitator is responsible for assisting with referral screening and conducting intake and service eligibility reviews and triaging clients to the most appropriate service. The Intake Facilitator is also responsible for providing Decision Guide assessment that address the psychosocial needs of the clients. The Intake Facilitator is an integral part of the interprofessional team including consultation with Coordinated Access and providing a clinical perspective when necessary. The Intake Facilitator will act as a liaison to internal and external programs within their scope of practice and participates in quality improvement activities within the care setting when requested.

**Education/Experience**:

- Minimum qualification includes undergraduate degree and registered member in good standing with applicable professional college (e.g. College of Nurses of Ontario, Ontario College of Social Workers and Social Service Workers, Ontario College of Psychologists, College of Registered Psychotherapists of Ontario, College of Occupational Therapists of Ontario)
- Knowledge and experience with referral processes, intake, assessment and triage
- Experience assessing clients with serious and complex mental health issues and making service recommendations
- Understanding of the social determinants of health of indigenous people and the social consequences and risk factors associated with client conditions (ie. addictions and mental health)
- Knowledge of community systems and resources for the purposes of networking between the facility/program and other services

**Skills/Abilities**:

- Ability to manage a caseload in a health care setting
- Ability to complete analysis, using clinical knowledge, skill and judgement to synthesize assessment information
- Assessment skills related to psychosocial issues
- Ability to communicate effectively and concisely, both orally and in writing
- Ability to work independently, interdependently, and participate as a contributing member of a variety of teams within the workplace, community and ministries/agencies
- Strong time management, analytical/critical thinking, problem solving and decision making skills
- Ability to use independent judgment and maintain confidentiality of records, actions, and information

**Assets**:

- Training and experience related to client care for people from Indigenous, geriatric, addictions and mental health, chronic pain populations would be an asset.

**CONDITIONS OF EMPLOYMENT**:
Must undergo a successful Police Vulnerable Sector Check (PVSC)
