**Shifts: Days**

Under the direct supervision of the Manager, Specialized Geriatric Services (SGS), the Registered Nurse - Intake Services is responsible for facilitating the entry of clients into the appropriate service within SGS and acts as the single-point of access. Key responsibilities include screening, assessing, and navigating clients to appropriate programs, and providing information about services.

**Qualifications**:
**Education/Experience**:

- Current unrestricted certificate of registration with the Ontario College of Nurses
- Valid CPR certificate
- Minimum of three years' experience providing health care services to the geriatric population
- Advanced knowledge and understanding of best practices related to dementia, delirium, mental health and its effect on the person and family members/care partners
- Demonstrated knowledge, understanding, and commitment to client-centred care
- Strong knowledge of community and regional services
- Knowledge and experience with various legislation including the Mental Health Act, Health Care Consent Act, and other relevant legislation

**Skills/Abilities**:

- Superior ability to communicate effectively and concisely, both orally and in writing and through a variety of means
- Effective listening, observation and facilitation skills to effectively assess the client and provide appropriate treatment and consultation
- Strong interpersonal skills to work with diverse client groups with varying levels of comprehension and language capability
- Ability to function and contribute as an effective team member and to work independently
- Strong leadership skills and demonstrated ability to organize, prioritize and multitask efficiently
- Ability to use independent judgment and maintain confidentiality of records, actions, and information
- Excellent computer, presentation and educational skills
- Demonstrated regular attendance at work

**Assets**:

- CNA Certificate in Geriatric Nursing
- Membership in professional association e.g. RNAO

**CONDITIONS OF EMPLOYMENT**:

- Must maintain certifications in good standing
- This position shall be conditional upon a satisfactory Police Vulnerable Sector Check (PVSC) where indicated, to ensure the absence of relevant criminal convictions.
