**Job Description Peer Support Coordinator (PSC)**

**Reports To**:

**Provincial Coordinator**: Peer Support Program Coordinator, Ontario Brain Injury Association (OBIA)

**Job Purpose**:
To provide overall coordination of the Peer Support Program for the Brain Injury Association __of_Thunder Bay ______________in collaboration with the OBIA.

The Peer Support program offers understanding, emotional, social and informational support between two people who share similar experiences after brain injury.

**Primary Duties and Responsibilities**:

- **Recruitment**:

- Recruit individuals who are living with the effects of acquired brain injury (survivors, caregivers, and other family members) to be Mentors and Partners within the program
- Promote and the Peer Support Program locally utilizing the promotional materials provided-may include but not limited to:

- Contacting and/or sending information to health providers such as doctors, physiotherapists etc.
- Ensuring that Peer Support materials readily available at local community association events (including support groups)
- Making other local social services aware of the program (i.e. mental health)
- **Screening and Intake**:

- Speak with potential Mentors and Partners about the program
- Using the tools provided, screen Mentors and Partners to ensure that their individual goals can be met through the program
- **Mentors**:

- Conduct an intake interview with a potential mentor (usually via telephone)
- Check references for potential mentors
- For recommended potential mentors enroll them in a scheduled mentor training with the Provincial Peer Support Coordinator
- **Partners**:

- Conduct an intake interview with a potential partner (usually via telephone)
- If partner is appropriate review the program, issues of confidently and Partner agreement and process of matching
- Provide resource information and alternative referrals for individuals that are not suitable or ready for the program
- **Matching and Support**:

- Facilitate matches (partnerships) between Mentors and Partners in a timely manner (within 2 weeks of the intake interview with a potential partner)
- Provide ongoing support to Mentors and Partners throughout their partnerships; offer regular follow-up and assist with resolution of problems as needed
- Support Post Partnership Evaluations at the completion of all Partnerships
- **Tracking and Reporting**:

- Ensure the timely completion of all activity logs including both Mentor and Coordinator Logs on the Peer Support Database
- Maintain accurate records of all Mentors and Partner information including Intakes, signed agreements and any program related communications.
- Provide monthly and quarterly reports to OBIA and the local BIA Board as required.
- **Mentor Training**:

- Co-facilitate Mentor Training (minimum one per year) with the support of the OBIA’s Peer Support Program Coordinator
- Provide ongoing support to mentors as needed
- Connect with OBIA’s Provincial Peer Support Coordinator for guidance and troubleshooting when necessary
- **Confidentiality**:

- Maintain confidentiality of personal information within the confines of the program
- **Other**:

- Forward recommendations for program improvements to the OBIA’s Peer Support Program Coordinator

Knowledge, Skills and Abilities:

- Knowledge of acquired brain injury and its consequences
- Knowledge of the principles of peer support
- Knowledge of local resources for people living with the effects of ABI
- Good written/verbal communication skills and organizational skills
- Willingness to commit up to a maximum of 75 hours per quarter (3 months)
- Ability to work flexible hours
- Ability to travel to attend one meeting per year (Annual PSP Meeting)
- Ability to adhere to local and provincial program reporting requirements
- Successful completion of Peer Support Coordinator Training with OBIA’s Peer Support Program Coordinator

**Job Type**: Part-time

**Salary**: $16.00-$23.00 per hour
