Position Information:
As part of the transformation underway in Ontario’s healthcare system, Oxford and Area organizations and community members have come together to form a Ministry of Health approved Ontario Health Team. The Oxford OHT is a group of health care providers that are working toward a shared vision of more integrated and better coordinated care. Through our collaborative partnership, the Oxford OHT will deliver on our shared purpose: Working together to build a stronger, healthier community for all.

Reporting to the Executive Project Lead, the Project Coordinator, Community Engagement will serve as a key member of our OHT Operations Team ensuring alignment with Oxford OHT strategic priorities. The individual will be responsible for supporting the design, development, and effective implementation of our community engagement strategy and initiatives. The Project Coordinator will engage with a range of community members - patients, clients, families, and caregivers - who are reflective of the Oxford and Area population and recognize and respect the diversity of our community. Through their engagement, they will seek opportunities to involve members of our community in the planning, design, delivery, and evaluation of integrated care, improved navigation, and innovation. The individual must be an exceptional communicator who will ensure that the lived and living experience is central to planning and decision-making.

RESPONSIBILITIES:

- Creates and implements a community engagement plan
- Align activities with the OHT’s attestation of the Patient, Family, and Caregiver Declaration of Values for Ontario
- Collaborates with Oxford OHT councils, committees, task forces, and working groups and ensures the community voice is represented at each table
- Supports the Patient Engagement Action Team chair, creates agendas, follows up on action items, documents decisions, and drives change
- Support the identification, recruitment, orientation, and ongoing support of community advisors
- Provides project management support including implementation, monitoring and control, risk identification, benefits realization, and budget management
- Conducts environmental scans and assessments of community based research, legislative and government requirements, and best practices
- Provides project leadership and project management support to the development and implementation pilot work plans; communicate project goals, objectives, and performance metrics; and document and track project phases and associated progress
- Works in partnership with Operations Team members to inform communication plans and provide regular, transparent, and accurate updates to key stakeholders
- Fosters partner engagement and nurtures and support change management while guiding their teams through complicated change
- Builds relationships with the community, stakeholders, patients and organizations as well as maintains and elevates existing relationships
- Supports OHT’s use of population health management and equity based approaches to improve care
- Monitors community engagement activities, contributes to reports and participates in the development of program evaluation projects
- Participates in virtual and face-to-face meetings

- Demonstrated ability to build partnerships and foster collaborative relationships
- Demonstrated effective presentation skills including the ability to present, train others, and discuss complex information in a way that establishes rapport, persuades others and gains understanding
- Strong ability to prioritize effectively and follow through on deliverables and deadlines to achieve successful results
- Effective time management skills and ability to balance competing demands
- Demonstrated ability to work with communications specialists to build out social media content about project successes
- Understanding of health care and social services in Oxford County

QUALIFICATIONS:

- Post-secondary education in core domains like business administration, health administration, child and youth work, planning, etc. or equivalent combination of education and work experience
- 3+ years of health and/or social service experience working in publicly funded organization/agency
- Project management, quality improvement, and/or change management experience, in health care setting
- Experience working in a wide
