**Change Coach Job Description**

Tall Tree Integrated Health Centre is seeking change coaches to join our growing health-care team in Victoria, BC.

**BASIC PURPOSE**

As a change coach, you are a vital part of the Tall Tree team. You are responsible for assisting our clients and patients to make and maintain changes to their health behaviour.

**Essential Duties**
- Work as part of a multidisciplinary health-care team focused on optimizing patients’ health and wellness:

- Collaborate with referring registered health professionals to define the referral issue and provide ongoing updates regarding patient progress
- Document clinical encounters in patients’ medical records
- Attend team meetings as needed
- Meet individually with referred patients to:

- Help them to identify areas for health behaviour change
- Develop detailed change plans
- Teach essential principles for successfully making and maintaining health-related changes over time
- Provide basic education on common behavioural risk factors and their links to common health conditions
- Link patients to broader resources as needed
- Provide motivation, guidance, support, and encouragement
- Adjust behaviour change plans over time

**Required knowledge, skills, and attributes**:

- Excellent verbal and written communication skills
- Excellent organizational skills
- Ability to work effectively as part of a diverse team
- Ability to work with a wide range of patients (diverse ages, socioeconomic backgrounds, presenting issues)
- Willingness to be flexible
- Commitment to clinical excellence, lifelong learning, and team-based care
- Previous education, training or certification in kinesiology or coaching.
- Previous education, training or certification in behavioural science, behaviour change, psychology, motivational interviewing, or habit change.
- Demonstrated knowledge of common diseases, illnesses, and conditions, and the typical medical treatments used to address them.
- Experience working directly with patients and/or clients.

**Job Types**: Part-time, Full-time

**Salary**: $23.00-$26.00 per hour
