Diversion Case Manager

We are searching for two part-time positions to work collaboratively to provide this high priority work with clients looking to access an emergency shelter bed. Shelter Diversion plays a significant role in preventing unnecessary entry into shelter, which then supports flow through and capacity within the emergency shelter system.
**Core Competencies**
**Key Responsibilities**:
- Meet with prospective clients entering the shelter to assess and explore safe alternative living arrangements through solution focused support and diversion services.
- Work within a system of care approach to assess diversion possibilities or entry to the shelter, and provide coordinated access to the shelter system if appropriate.
- Complete thorough intakes for any clients who are not successfully diverted to promote a smooth flow experience for clients and to support to staff on the shelter floor.
- Provide conflict resolution and mediation to clients, their supports and/or landlords to prevent homelessness.
- Develop and maintain positive working relationships with community partners and support clients with referrals to appropriate professional services.
- Assist cross-teams to creating practices and protocols that promote positive shelter diversion outcomes.
- Participate in client care reviews and team meetings to ensure effective delivery of support services and continuity of care.
- Utilize assessment tools (VI-SPDAT) to provide accurate care planning, outcomes and client centered goal development.
**Administrative Duties**
- Compiles statistical information and prepares monthly reports as required for program evaluation and development.
- Maintain accurate records
**Job Types**: Part-time, Fixed term contract
Contract length: 10 months
Pay: From $25.50 per hour
Expected hours: 18 per week
