To help fulfill its mission, LOFT is hiring a **Full-time Contract Case Manager**
**Job Summary**
As a Case Manager you will work to coordinate a support network for clients living with mental health challenges, substance use issues, physical health issues, and risks of housing loss. The position attempts to empower the client, thereby increasing their independence, dignity and self-respect. Direct support will be provided to clients in service navigation and advocacy, linkages to physical and mental health care, goal setting, crisis intervention, and housing and shelter access.
The **McEwan Mental Health Team**provides Case Management for adults 16 or older living with HIV or AIDS who experience mental health challenges and/or substance use issues in the Toronto core, supporting health care and system navigation, and ensuring inclusiveness of 2SLGBTQ+ and BIPOC communities, with the goal of community re-integration and personal empowerment.
**What You Will Do**
- Develop a plan of support and recovery which includes identifying priorities and goals, in conjunction with client, other professionals, areas identified through initial intake and OCANs;
- Promote the personal empowerment of each individual through coaching, mentoring, support and follow-up in the development of life skills, including maintaining housing, setting and attending appointments, navigating services, and managing activities of daily living;
- Establish and co-ordinate a support network of social, family, medical, psychiatric and other community links. Develop a coordinated care plan as appropriate;
- Support in client’s long-term housing stabilization, focusing on identifying, recommending and referring to various housing supports, including hoarding support, personal care support, external support for pest control treatment preparation, addressing tenancy rights and responsibilities, and other needs as appropriate;
- Mediate issues or conflicts between residents and neighbours, teaching conflict resolution; following-up on issues to ensure resolution;
- Participate in planning, organising, promoting and conducting events for the clients in an effort to address their social needs and prevent isolation; maintain a general awareness of community events and activities to also meet these needs;
- Participate in the administration of the program by completing and maintaining the required records, assessments or statistics such as client files in accordance with established procedures;
- Participate as a team member in program delivery by providing support and consultation to colleagues, identifying gaps in service and recommending solutions, participating in meetings, planning and evaluation initiatives, committees, events and activities; orienting new staff, students or volunteers; providing other related assistance as required.
**What You Bring**:
Must Haves
- An excellent understanding of harm reduction, and psychosocial rehabilitation principles as acquired through a relevant degree/diploma in human services
- At least three years of experience working with individuals living with HIV/AIDS who have experienced homelessness and mental health and/or substance use barriers
- At least three years of experience supporting individuals in 2SLGBTQ+ communities with demonstrated skill in working with trans, non-binary and gender fluid individuals, including knowledge of relevant community services
- An understanding of the therapeutic relationship with an ability to negotiate with clients on goals and priorities
- Strong skills in working within formal partnerships, including maintaining timely and thorough communication, collaborative problem-solving, and maintaining strong inter-agency relationships
- Excellent interpersonal, relationship-building, communication and counselling skills
- Understanding of anti-oppressive principles and trauma-informed care
- Good analytical, time management and organizational skills to maintain consistency in client support
- Good intake assessment, crisis intervention, advocacy and problem-solving skills
- Computer literacy sufficient to produce and maintain files, records or statistical information
