**Fife House** is an innovative, client-focused provider of secure and supportive affordable housing and services to people living with HIV/AIDS in the Greater Toronto Area. With approximately 110 staff working across 6 locations, in 2020-2021 Fife House served more than 850 residents and clients by providing supportive and transitional housing programs as well outreach programs to homeless clients and their families.

ABOUT THE ROLE:
Fife House’s Linkage to Care & Satellite Sites program provides culturally-responsive and strengths-based intensive case management to individuals living with HIV, who have identified support needs relating but not limited to: physical health, medication access, mental health, substance use, criminal justice involvement, immigration, and/or precarious housing with the goal of improving housing stability. The program prioritizes Black, Indigenous, and Persons of Colour (BIPOC) living with HIV and is committed to the advocacy and support of racialized community members navigating complex systems of care. The program consists of Case Managers, Housing Stabilization Workers, and Community Navigators. The case manager position provides one-on-one case management through client-directed care plans which will connect individuals to the services that they need. The case manager will provide support to multiple subprograms (Housing Focused Client Supports, Houseless Outreach Program, & Newcomer Short-term Supports Program) as well as internal Fife House residential programs, including Huntley Transitional Housing Program, Sherbourne Apartments, Sherbourne Transitional Housing Program, Denison, Jarvis, Sorauren, and other sites as identified. The position directly supports clients who are experiencing houselessness, living in transitional housing, at risk of eviction, and/or moving into a permanent housing unit. The case management will work primarily in community, including outdoors, shelters, client rooms/apartments, clinics, and social service agencies, in all weather conditions. The goal of services is to facilitate access to support services via referrals, provide complex systems navigation, and increase clients’ competency to advocate for themselves using a psychosocial rehabilitation approach to care.
**DUTIES AND RESPONSIBILITIES**:
- Provide comprehensive culturally-responsive and community-based case management services, needs assessments, service navigation support and care plan development to a diverse caseload of up to 30 clients;
- Complete client intakes for approved referrals and coordinate client enrollment documentation;
- Participate in workshops, training and meetings facilitated by the City of Toronto as per funding requirements;
- Engage effectively with Black, Indigenous and Persons of Colour experiencing mental health, addictions, cognitive issues, physical health issues and/or homelessness;
- Use a harm reduction and strengths-based approach in service provision, acknowledging historical mistrust and fear of the target population in accessing services;
- Use an Intensive Case Management approach for clients with high support needs, including contacts 2-3x week;
- Advocate on behalf of clients to access a range of services;
- Conduct outreach in the community, including visits to client units, encampments, parks, shelters, and other sites;
- Participate in joint case conferences and use a collaborative approach to coordinate access to services and facilitate referrals, such as housing, identification replacement, legal, financial assistance, primary care, HIV specialists, mental health and addiction supports, emergency shelters, etc.;
- Collaborate with internal Fife House residential programs to facilitate joint case management meetings and regular communications in regards to client support;
- Provide appointment accompaniment where necessary to promote housing stability and liaise with relevant service providers to coordinate supports;
