To help fulfill its mission, LOFT is hiring a **Case Manager.
**Job Summary**
The Case Manager on the M-DOT Hotel Team provides direct service to homeless individuals living with mental health, health, and addictions issues who may currently be experiencing or have previously experienced homelessness or precarious housing. The position provides both on site and community-based assessment and case management services to individuals living in various physical distancing shelter hotel programs through the city of Toronto. Case Managers work as part of a team that includes Team Leaders, Coordinators, Peer Support Workers, and Housing Support Workers, to meet clients’ housing and other social and health needs. The program works closely with nurses and physicians from Inner City Health Associates to assess, plan, treat and support clients. The position will involve driving and a valid Ontario driver’s license and insurance are required. Access to a car is a requirement.
**M-DOT Hotel** is a specialized team of providers that delivers services to the most vulnerable individuals accessing shelters and Drop Ins across the City of Toronto. M-DOT Hotel is comprised of team leads, a housing case manager and mental health case managers who work out of various shelters and/ or hotels across the City of Toronto. Additionally, M-DOT Hotel, works very closely with our partners at each shelter/ hotel (operators) and ICHA, who provides Primary Care Led Teams, providing psychiatry and health consultation and Parkdale Queen West CHC, who provides Harm reduction and addiction supports at each site. M-DOT Hotel provides clinical hands-on support to individuals often identified as ‘complex’, who have not responded positively to other programs or attempts to engage. M-DOT Hotel provides outreach and case management services to provide consultation, access to treatment, service planning and advocacy. Services are designed to be short-term and mobile, following individuals through different services and stages of their homelessness, focused on providing access, stabilization and linking to long term resources. The aim of M-DOT Hotel is to provide a low barrier transitional service, that meets individuals where they are at and assists in stabilizing their health, mental health and substance use. M-DOT works from a housing first and harm reduction approach. The goal is to meet immediate needs, reduce barriers and assist individuals in getting connected to housing and follow up support that meets the level of need required for each individual to live successfully in the community.
**_Note:_**_ The Housing Case Manager is expected to meet clients outside of the office. Visits are provided in the clients’ homes or other locations in the community, which requires physical activities such as significant driving, frequent stops, getting in and out of the car and climbing stairs. Accompanying clients to appointments is an expectation of the position._
**What You Will Do**
- Engages effectively with individuals experiencing serious mental health issues and/or addictions, cognitive difficulties, brain injury or primary health care issues
- Conducts outreach shifts independently or with other team members as scheduled
- Develops relationships with individuals served by the team and addresses basic and immediate needs
- Engages in crisis intervention/prevention with clients
- Case manages from a psycho-social rehabilitation and recovery model, linking clients with other supports as necessary, including housing, transportation, shelter placement, identification (ID) services and translation services, among others
- Develop relationships with external housing partners to build partnerships around access to housing
- Complete housing need assessments with clients
- Liase and support your colleagues around housing resources
- Liaises with Ontario Works (OW) and the Ontario Disability Support Program (ODSP) as required.
- Works from a harm reduction approach with clients dealing with addiction issues
- Advocates on behalf of homeless individuals in accessing a range of services
- Works cooperatively with team members, hospital personnel, other service providers and City staff in order to better meet client’s needs
- Facilitates transfer of cases to Streets to Homes funded follow-up support services or ongoing case management services
