**Job Description**:
Reporting to the Manager of the Hematology program, the **Coordinator, Complex Malignant Hematology (CMH)** will be the primary coordinator for patient care that is delivered both within the CMH program and in collaboration with the shared care, leukemia and Cell therapy teams at Princess Margaret (PM) Cancer Centre.
The Coordinator, CMH will facilitate communication between the CMH/malignant hematology team, inpatient care unit, consultants, other RVH departments and SMRCP leadership. Enabling and role modeling teamwork, communication, autonomous clinical practice and a drive towards continuous program and team improvement, the individual will work alongside the interprofessional team to optimize the patient experience, improve patient flow and achieve quality patient outcomes.
This position will collaborate with internal and external stakeholders to coordinate patient care as well as develop, implement and sustain best practices to improve quality of care and efficiencies in the CMH program.
**Education**:
- Baccalaureate Degree in Nursing required
- Current Registration with the College of Nurses of Ontario required
- Current Provincially Recognized Chemotherapy/Biotherapy certification or maintenance course (ONS or de Souza Institute)
- Master’s level preparation in a health profession or related field an asset
- Current BCLS, Level II Rescuer certification required
- Canadian Oncology Nurse Certification considered an asset
- Membership to Oncology related association considered an asset
- Successful completion of de Souza Complex Malignant Hematology course required
**Experience**:
- Three (3) years current or recent malignant hematology oncology experience required
- Recent systemic therapy/chemotherapy/biotherapy administration experience an asset
- Extensive knowledge of malignant hematology disease and treatment trajectories required
- 1 - 2 years experience working in an advanced clinical practice role preferred
- Previous project management and leadership experience
- Participation in policy development and process improvement initiatives required
