Drive Clinical Excellence and Regional Impact

Join a dynamic team at the forefront of advancing palliative care across the region. As a Palliative Care Implementation Specialist, you will play a critical role in supporting the Lead - Palliative Care Implementation and the Regional Implementation team in delivering a high-priority, multi-year initiative. Where Indigenous communities wish to be involved, they will provide support to Indigenous communities and Indigenous organizations for their implementation efforts. This position will be regional in nature, focusing on pertinent needs of the Northwest Region.

Here is what you will be doing:
- Collaborative Project Support: Work closely with senior leaders and multidisciplinary teams to facilitate the successful implementation of region-wide palliative care initiatives.- Strategic Planning & Execution: Evaluate, revise, and assist in carrying out comprehensive plans to ensure all project accountabilities are met efficiently.- Project Management: Utilize best-practice project management processes and tools to monitor progress and keep projects on track.- Monitoring & Risk Management: Conduct ongoing status reviews, identify and escalate key issues or risks, and propose practical solutions to mitigate challenges.- Data-Driven Improvement: Implement data collection tools across organizations, leveraging data and evidence to drive clinical quality improvements and support continuous learning and advancement in care delivery.- Reporting: Draft clear, informative reports detailing implementation progress and maintain documentation to support effective project management and transparency.- Communication & Stakeholder Engagement: Present complex ideas in a clear, compelling manner using strong written, verbal, and graphical skills, ensuring information is accessible to diverse audiences.- Education & Collaboration: Partner with regional and local clinical experts to coordinate and support palliative care education initiatives, fostering engagement and knowledge sharing throughout the region.
- Here is what you will need to be successful:
Education and Experience- A Bachelor’s degree is required, with a preference for a degree in health science, health administration, business administration or a related discipline.- A Master’s degree in one of the disciplines above is an asset.- Project management certification is an asset (i.e., PMP).- Three (3) years of project management experience is required, with a strong preference for project experience within the health care sector.- Experience with palliative care initiatives is an asset.- Experience with community health organizations or long-term care homes is an asset.- A strong understanding of regional priorities and local health care needs, within Ontario’s health care landscape.

Knowledge and Skills- A commitment to developing a coordinated and standardized approach for delivering palliative care services for all people in Ontario, regardless of their age or illness.- Knowledge of relevant legislation, including the Fixing Long-Term Care Homes Act, and the Excellent Care for All Act.- Strong planning and project management skills.- Ability to successfully execute large-scale, complex initiatives.- Excels in a fast-paced and objective-based environment.- Superior attention to detail with excellent organization and time management skills.- Able to prioritize, effectively antic
