**Rate of Pay: $38.682 to $48.359 per hour**

Reporting to the Manager - Regional Palliative Care Program, the Regional Palliative Care Clinical Coach is responsible for providing palliative care coaching and mentoring to staff and clinicians to build primary-level palliative care competencies. With a focus on community service providers and primary care in the district of Thunder Bay, the palliative care clinical coach will work collaboratively with community organizations in the district to facilitate the implementation of the Palliative Care Health Services Delivery Framework in Community. The Palliative Care Clinical Coach will support practice change at the organizational level through engagement, quality improvement, change management and evaluation. At the clinician level, the Palliative Care Clinical Coach will support practice change through mentorship, coaching education and clinical consultation.

**Qualification**:
Education/Experience:

- A bachelor's degree in a registered health care discipline (e.g., RN, RSW)
- A minimum five (5) years' experience in practice in palliative care is required
- Experience working with large-scale quality improvement initiatives
- Experience working with a range of roles, disciplines, and levels of staff in different health care settings

Skills/Abilities:

- Ability to develop relationships based on trust, compassion, and empathy with a demonstrated focus on person-centred care
- Strong active listening and interpersonal communication skills for one-on-one coaching
- Experience designing education to effectively address palliative care training needs of interprofessional learners
- Excellent communication skills and experience with culturally responsive teaching
- Supports the advancement of equity, inclusion, and diversity across Ontario's health care system
- Demonstrated commitment to engage with First Nations, Indigenous, Métis and urban Indigenous people as they self-determine their own approach to palliative care
- Proficiency in educational technologies (e.g., webinars, apps) as well as knowledge of Microsoft Outlook, Word, Teams, PowerPoint, and Excel
- Knowledge of relevant legislation and reporting requirements, including the Fixing Long-Term Care Homes Act, the Excellent Care for All Act and the Compassionate Care Act and other pertinent legislation and regulations related to professional practice is required
- Strong clinical coaching and debriefing skills, with both individuals and teams
- Ability to effectively prioritize workload to meet various deadlines
- Strong facilitation and consensus building skills
- Proven problem solving, analytical and evaluation abilities

**CONDITIONS OF EMPLOYMENT**:

- A valid Ontario driver's license with the ability to travel to various work locations and communities within the district
- Will be required to travel to various work locations throughout Northwestern Ontario and provide own reliable transportation
