**Job Description**:
Reporting to the Director of Professional Practice, IPAC & Allied Health Services the Clinical Practice Coordinator
- Expanse will be responsible for the coordination of the Expanse Support Team with a focus on building quality outcomes. The Clinical Practice Coordinator
- Expanse will role model teamwork, communication, and a drive towards continuous improvement, the individual will work alongside the interprofessional team to champion the identification of quality initiatives aimed at optimizing the patient experience and improving support for staff in achieving quality patient outcomes.

In collaboration with the Professional Practice program and other clinical programs, the Clinical Practice Coordinator
- Expanse will provide effective leadership for the Expanse Support Team within the department, as well as support the individual needs of each program throughout the organization. Working with the interprofessional team the individual will be responsible for assisting with daily staff development, communicating change ideas and sustaining quality initiatives to meet the program goals and objectives.

This position will collaborate with internal and external stakeholders to develop, implement and sustain best practices to improve quality of care and care models through the use and optimization of the Meditech Expanse system.

**Education**:

- Baccalaureate Degree in a health related study required, Masters in a health related study preferred
- Specialty Certification in quality improvement preferred
- Formal education in leadership and/or management essential
- Previous project management and/or change management experience and participation in process improvement initiatives required

**Experience**:

- Minimum 2 years current or recent leadership experience required
- Demonstrated quality improvement experience required
- Demonstrated change management experience required

**Competencies**:

- Demonstrated ability to communicate effectively. Excellent verbal and written communication skills
- Strong interpersonal skills
- Project Management skills including facilitation skills preferred
- Leadership ability
- Ability to problem solve clinical issues with the support of the program leadership team

- Demonstrates knowledge of patient and staff safety policies and procedures

**Other**:

- Working knowledge and understanding of Patient Information systems; preference given to those with Meditech Expanse experience.

- Ability to learn about, from and with other members of the patient care team to foster a strong interprofessional model of care
